摘要
目的:统计并分析我院静脉用药集中调配中心接收的神经外科不合理医嘱,为药师提供审方参考,也为神经外科临床的合理用药提供依据。方法:依据2015版《中华人民共和国药典》、《新编药物学》、《处方管理办法》、《药物临床信息参考》、《临床静脉用药调配与配伍速查》、《432种静脉注射剂配伍指南》及药品说明书等资料,统计并分析我院神经外科2016年6月—2017年5月之间静脉用药集中调配中心接受的不合理医嘱。结果:神经外科一年36990份处方中共包含不合理医嘱238份,占医嘱总数的0.64%。所涉及的不合理医嘱处方内容主要包括药物选择不适宜、用法用量不适宜、给药途径不适宜、联合用药不适宜、重复给药、有配伍禁忌或不良相互作用、溶媒不适宜、医嘱录入错误等问题。结论:审方药师在合理用药过程中发挥着重要作用,通过审方药师干预不合理处方,可进一步确保神经外科的用药安全。
To analyze the neurological irrational doctor's orders of pharmacy intravenous admixture services(PIVAS)received in our hospital,and to provide reference and a basis for pharmacists and clinical neurosurgery rational drug use respectively.Methods:According to the2015edition of"Pharmacopoeia of the People's Republic of China","New Pharmacology","Prescription Management Regulations","Clinical Drug Information Reference","Clinical intravenous drug use Quick deployment and compatibility,""432kinds of intravenous injections Compatibility Guide"and the Drug Instructions,the irrational Neurosurgery Division prescription of PIVAS received from June in2016to May in2017were statistically classified and analyzed.Results:There were238irrational prescriptions(accounted for0.64%)in36990prescriptions.The inconvenience of the prescriptions involved mainly include:the selection of drugs is not appropriate;the usage is not appropriate;the route of administration is not appropriate;combined with the drug is not appropriate;repeated administration,with contraindications or adverse interactions;the solvent is not appropriate and other issues.Conclusion:The prescription pharmacists play an important role in the course of rational drug use.Through the prescription intervention of pharmacist,the neurosurgery drug safety was further insured.
