2例晚期妊娠合并肝癌患者的围术期护理配合体会

Perioperative nursing experience of two pregnant women with liver cancer in the third trimester

目的总结2例晚期妊娠合并肝癌患者外科治疗的围术期护理配合要点。方法术前参与手术讨论,做好人员、环境和物品、器械和设备准备,协助医生先在硬膜外麻醉下行剖宫产术,婴儿娩出后,在静吸复合全身麻醉下行肝癌切除并胆囊切除术。结果 2例患者术程顺利,新生儿存活,母婴被成功救治,手术时间平均6 h,出血平均2000 ml。均于术后9天出院。结论孕期一旦确诊肝癌,应尽早行外科手术切除肿瘤并终止妊娠。术前充分准备,术中娴熟准确的护理配合是手术成功的有力保障。

Objective To summarize the perioperative nursing key points of two cases pregnant women with liver cancer in the third trimester. Methods Before surgery, surgical nurses participated in preoperative discussion and prepared nursing staffs, operation theaters, supplies and instruments for the surgery.On the day of surgery, surgical nurses assisted cesarean section under epidural anesthesia and then hepatec-tomy and cholecystectomy under combined general anesthesia. Results Surgeries were both completed smoothly with no related death. The mean operation time was 6 hr, with a mean blood loss of 2000 ml. Puerperae were both discharged on postoperative day 9. Conclusion Once the diagnosis of liver cancer was confirmed in pregnant women, tumor should be resected and pregnancy terminated promptly. Thorough preop-erative preparation combined with skilled and accurate nursing cooperation during surgery is the key to suc-cessful operation.
