达托霉素治疗糖尿病足伴感染一例

Use of daptomycin in diabetic foot infection: a case report

本例糖尿病足伴感染患者同时合并糖尿病视网膜病变、糖尿病周围神经病变、糖尿病肾病3种慢性并发症,临床医师经验性予替考拉宁及亚胺培南西司他丁钠抗感染治疗后疗效不佳,患者并发脓毒性休克,后改用达托霉素抗感染治疗,并通过积极抗休克、多次清创引流、血液净化、营养支持等治疗,最终患者糖尿病足伴感染得到有效控制。

In this case,the patient admitted to our hospital with a diabetic foot infection.He was diagnosed as diabetes complicated with diabetic retinopathy,diabetic peripheral neuropathy and diabetic nephropathy.The initial empirical antibiotics were teicoplanin combined with Imipenem/Cilastatin Sodium but without clinical effect.The patient was worsened and complicated with septic shock.We changed antibiotic to daptomycin,together with shock resuscitation,debridement and drainage,blood purification and nutrition support,and finally we cured the diabetic foot infection.
