PFNA内固定联合中医综合疗法治疗老年股骨粗隆间骨折48例的临床观察

Clinical observation on treating senile intertrochanter fracture of femur by PFNA internal fixation plus TCM comprehensive therapy

目的:探讨PFNA内固定联合中医综合疗法治疗老年股骨粗隆间骨折的临床效果。方法:将2016年1-12月收治的48例老年股骨粗隆骨折患者随机分组,对照组采用防旋股骨近端髓内钉(PFNA)进行治疗,观察组采用PFNA联合中医综合疗法进行治疗。比较两组康复效果、不良反应与手术指标等。结果:(1)和对照组(66.67%)对比,观察组的治疗总有效率(95.83%)明显更高,数据比较差异显著,P<0.05。(2)和对照组对比,观察组生活质量评分明显更高,即观察组中总体健康情况评分为(88.29±2.55)、身体疼痛为(85.50±3.33)、活力为(90.35±4.55)、生理功能为(86.23±4.60)、生理职能为(86.30±5.19)、社会功能为(90.25±3.25)、精神健康为(91.35±3.20)、情感职能为(90.96±3.30),P<0.05。观察组治疗后生活质量评分明显高于治疗前,P<0.05。(3)和对照组(33.33%)对比,观察组不良反应发生率(4.17%)明显更低,数据比较差异显著,P<0.05。(4)与对照组对比,观察组平均住院时间为(6.30±1.50)周,平均骨折愈合时间为(3.20±0.55)个月,明显更短,数据比较差异显著,P<0.05。结论:PFNA内固定联合中医综合疗法治疗老年股骨粗隆间骨折的临床效果显著,可有效地促进骨折部位愈合,有利于改善患者的预后情况,值得临床广泛应用。

Objective:To investigate the clinical effect of PFNA internal fixation plus TCM comprehensive therapy on intertrochanteric fractures of the elderly.Methods:48patients were randomly divided into two groups.The control group was treated by PFNA.The observation group received TCM comprehensive therapy more.Results:The total efficiency was66.67%in the control group,less than95.83%in the observation group(P<0.05).The quality scores in the observation group were significantly higher than the control group and before treatment,P<0.05.Adverse reaction was33.33%in the control group,significantly more than4.17%in the observation group(P<0.05).The average time of hospitalization was(6.30±1.50)weeks;and average fracture healing time was(3.20±0.55)months;those in the observation group were shorter than the control group.Conclusion:PFNA internal fixation plus TCM comprehensive therapy on intertrochanteric fractures of the elderly was effective,by promoting the healing of fracture site and improving the prognosis,and is worthy of clinical application.
