摘要
目的:了解我市三级甲等公立医院护理纠纷案例,分析产生的综合原因。方法:整群分层随机抽样的方法调查我市三级甲等公立医院护理纠纷案例,分析不同情况下产生的相关护理纠纷案例,从而分析导致此现象出现的原因。结果:68例医疗纠纷主要原因依次为:服务意识差23例(33.8%)、护理措施不到位22例(32.4%)、执行医嘱不当14例(20.6%)、违反操作规程7例(10.3%)、擅离职守2例(2.9%);调查对象的工作年限(χ~2=45.31,P=0.00)、护理人员年龄(χ~2=81.31,P=0.00)、所在科室(χ~2=51.81,P=0.00)对护理纠纷产生的影响有统计学意义;若干影响因素对护理纠纷发生影响的综合分析,回归模型有统计意义,Logit P=-3.42+0.21X1+0.14X2+0.11X3+0.31X4+0.64X5+0.31X6+0.54X7+0.47X8,影响大小前3位依次为:服务意识差Exp(B)=1.55、护理措施不到位Exp(B)=1.19、科室Exp(B)=1.15。结论:护理人员的人口学因素对护理纠纷发生有影响,纠纷原因主要为:服务意识差、护理措施不到位、执行医嘱不当。
Objective:To learn about the nursing dispute cases in the city's tertiary level public hospitals and analyze the causes of nursing dispute cases comprehensively.Methods:The method of stratified cluster random sampling survey of the city's tertiary level public hospital nursing dispute cases was carried out and analysis of relevant nursing disputes arising under different circumstances was made in addition to analyzing the causes of this phenomenon.Results:The main reasons of68cases of medical disputes included:poor sense of service in23cases(33.8%),wrong nursing measures in22cases(32.4%),inappropriate nursing in14cases(20.6%),clinical mishandling in7cases(10.3%),absence without leave in2cases(2.9%);the working years(χ2=45.31,P=0.00),age of nurses(χ2=81.31,P=0.00),sections(χ2=51.81,P=0.00)were statistically significant for the nursing effect of arising disputes;Comprehensive analysis of some influencing factors on nursing disputes was made,the regression model had statistical significance,Logit P=-3.42+0.21X1+0.14X2+0.11X3+0.31X4+0.64X5+0.31X6+0.54X7+0.47X8,the top3affecting factors were:poor sense of service Exp(B)=1.55,wrong nursing measures Exp(B)=1.19,departments Exp(B)=1.15.Conclusion:The demographic factors of nursing staff had an effect on the occurrence of nursing disputes,which were mainly caused by poor service consciousness,inadequate nursing measures and improper execution of doctors'orders.
