透明质酸在控制妇科盆腔手术粘连发生及腹腔液粘连相关指标中的效果观察

Observation on the effect of HA in controlling the adhesion and peritoneal fluid adhesion related indexes of patients with pelvic operation

目的观察探讨透明质酸(HA)在控制妇科盆腔手术粘连发生及腹腔液粘连相关指标中的效果。方法选取2014年9月~2015年12月于本院进行盆腔手术治疗的400例患者为研究对象,根据随机数字表法将其分为对照组(常规干预组)200例和观察组(HA应用组)200例,然后将两组中不同类型患者的粘连发生率及手术前、手术后的7、14及28d腹腔液粘连相关指标进行统计与比较。结果观察组中初次再次手术患者的粘连发生率(1.86%,5.13%),均明显低于对照组同类手术患者(9.26%,23.68%),差异有统计学意义(P=0.004,0.020)。除此之外,手术后7、14及28d,对照组患者的腹腔液LN、PCⅢ、PⅢP及FN水平均小于观察组(P<0.05)。结论 HA可有效控制妇科盆腔手术粘连的发生,且对腹腔液粘连相关指标也有积极的改善作用,临床综合应用价值较高。

Objective To observe and study the effect of hyaluronic acid(HA)in controlling the adhesion and peritoneal fluid adhesion related indexes of patients with pelvic operation.Methods Four hundred patients with pelvic operation in our hospital from September2014to December2015were selected as research object,and they were divided into control group(conventional intervention group)200cases and observation group(HA application group)200cases according to the method of random digits table,then the adhesion rate of different operation types and peritoneal fluid adhesion related indexes before the operation and at7th,14th and28th day after the operation of two groups were analyzed and compared.Results The adhesion rates of observation group with first operation(1.86%)and reoperation(5.13%)were all lower than those of control group with same operation method(9.26%and23.68%),there were statistical significance(P=0.004,0.020).The peritoneal fluid adhesion related indexes such as LN、PCⅢ、PⅢPand FN at7th,14th and28th day after the operation were all obviously lower than those of control group tool(P<0.05),there were significant differences.Conclusion The HA can effectively control the adhesion rate,and it also plays an active role for the improvement of peritoneal fluid adhesion related indexes of patients,its clinical comprehensive application is higher.
