综合护理干预对颅脑外伤患者开颅术后颅内及其他感染发生率的影响

Effect of comprehensive nursing intervention on incidence of intracranial and other infections in patients with craniocerebral trauma after craniotomy

目的探讨综合护理干预对颅脑外伤患者开颅术后颅内及其他感染发生率的影响。方法选取70例行开颅手术治疗后的颅脑外伤患者为研究对象,按主诊医生分管病区不同分为两组,每组35例。对照组和研究组分别实施常规护理和综合护理干预。比较两组患者颅内感染及其他感染发生率、术后半年和1年患者生存率以及两种护理模式的患者满意度,对比两组护理质量评分以及生理健康评分(APACHE-Ⅱ)。结果与对照组比较,研究组患者颅内感染等并发症发生率较低(8.6%vs20.0%),对护理模式满意度更高(94.3%/82.9%),差异均有统计学意义(P<0.05)。研究组患者半年和一年生存率均高于对照组(100%vs91.4%,94.3%vs85.7%),差异有统计学意义(P<0.05)。结论与常规护理相比,颅脑外伤患者开颅术后实施综合护理,能更有效地减少颅内及其他各类感染的发生,并可提高患者对护理模式的满意度以及术后生存率。

Objective To investigate the effect of comprehensive nursing intervention on the incidence of intracranial and other infections in patients with craniocerebral trauma after craniotomy.Methods Seventy cases of craniocerebral trauma patients after craniotomy were selected as the research objects.The patients were divided into two groups according to the division of the main doctor,with35cases in each group.The control group and the study group were given routine nursing and comprehensive nursing intervention respectively.The incidence of intracranial infection and other infections,postoperative survival rate of six months and1years,and patients'satisfaction of two nursing modes were compared between the two groups,and the nursing quality scores and physical health scores(APACHE-Ⅱ)were compared between the two groups.Results Compared with the control group,the incidence of intracranial infection and other complications in the study group was lower(8.6%vs20.0%),and the satisfaction of nursing mode was higher(94.3%vs82.9%),the difference was statistically significant(P<0.05).The half year and one year survival rate of the patients in the study group were higher than those of the control group(100%vs91.4%,94.3%vs85.7%),and the difference was statistically significant(P<0.05).Conclusion Compared with routine nursing,comprehensive nursing after craniotomy in patients with craniocerebral trauma can reduce intracranial and other kinds of infections more effectively,and improve patients'satisfaction with nursing mode and postoperative survival rate.
