输尿管软镜下碎石术与微通道经皮肾镜碎石术治疗肾下盏结石的疗效分析 被引量:8

Analysis on curative effect of flexible ureteroscopic lithotripsy and microchannel percutaneous nephrolithotomy for treating subrenal calyx calculus

摘要 目的对比分析输尿管软镜下碎石术(FURL)与微通道经皮肾镜碎石术(MPCNL)2种手术方式治疗肾下盏结石的安全性及有效性。方法回顾性分析该院2015年1月至2017年1月采用FURL(FURL组,40例)或MPCNL(MPCN组,56例)治疗肾下盏结石96例患者的临床资料。比较2组患者结石清除率、并发症发生率、手术时间、术后住院时间和血红蛋白水平下降情况等。结果 2组患者结石清除率、术后发热发生率、输血率比较,差异均无统计学意义(P>0.05);FURL组手术时间、术后住院时间均短于MPCNL组,而术后血红蛋白下降水平低于MPCNL组,差异均有统计学意义(P<0.05)。结论 FURL与MPCNL治疗肾下盏结石均安全、有效,与MPCNL相比,FURL具有损伤小、出血少、恢复更快等优点。 Objective To comparatively analyze the safety and effectiveness of flexible ureteroscopic lithotripsy(FURL)and microchannel percutaneous nephrolithotomy(MPCNL)in the treatment of subrenal calyx calculus.Methods The clinical data of96patients with subrenal calyx calculus treated with FURL(40cases)or MPCNL(56cases)in this hospital from January2015to January2017were retrospectively analyzed.The stone clearance rate,incidence rate of complications,operation time,postoperative hospitalization time and hemoglobin decrease were compared between the two groups.Results There was no statistical difference in the stone clearance rate,incidence rate of postoperative fever and postoperative blood transfusion rate between the two groups(P>0.05);the operative time and hospitalization time after operation in the FURL group were shorter than those in the MPCNL group,while the decrease level of postoperative hemoglobin was lower than that in the MPCNL group,the difference was statistically significant(P<0.05).Conclusion Both FURL and MPCNL are safe and effective in the treatment of subrenal calyx calculus.Compared with MPCNL,FURL has the advantages of small trauma,less bleeding and fast recovery.
