家庭肠内营养对食管癌根治术后患者营养状况的影响研究 被引量:16

摘要 目的探讨家庭肠内营养对食管癌根治术后出院患者营养状况的影响。方法选取2016年1月—2017年3月符合纳入标准的85例早期行食管癌根治术并空肠造瘘术的患者作为研究对象,随机分为干预组44例与对照组41例。干预组出院后经营养评估,除经口进食普通居家膳食外,在专业营养支持小组的指导下给予肿瘤专用免疫型肠内营养制剂经空肠造瘘管饲;对照组出院后经营养评估,常规经口进食普通居家膳食,空肠造瘘管夹管。监测两组体质量指数(BMI)、欧洲营养风险指标(NRS 2002)及实验室血清营养指标,统计分析两组基线水平和干预1个月后各指标的差异。结果 1个月后,干预组BMI高于对照组;欧洲营养风险指标干预组低于对照组;实验室血清营养指标优于对照组,差异均具有统计学意义(P<0.05)。结论食管癌根治术后开展家庭肠内营养支持,可明显改善患者的营养状况,降低术后营养不良风险,提高患者满意度。 Objective To investigate the effect of family enteral nutrition on the nutritional status of discharged patients after radical resection of esophageal cancer.Method85patients with early esophageal cancer plus jejunostomy met the inclusion criteria from January2016to March2017were selected and randomly divided into intervention group(44cases)and control group(41cases).The intervention group was evaluated by nutritional assessment after discharge.The special immuno enteral nutrition preparations were given by jejunostomy tube under the guidance of the professional nutrition support group.The control group was evaluated by nutrition after discharge.Routine oral meals were given routinely diet and jejunostomy tube.The body mass index(BMI),European nutrition risk index(NRS2002)and laboratory serum nutrition index were monitored and compared.The difference between two groups baseline level and1months after intervention were statistically analyzed.Result The baseline level of the two groups was not statistically significant(P>0.05).After1months,the BMI in the intervention group was higher than that of the control group,and the European nutritional risk index in the intervention group was lower than that of the control group.The serum nutrition index in the intervention group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Family enteral nutrition support after radical resection of esophageal cancer can significantly improve the nutritional status of patients,reduce the risk of postoperative malnutrition and improve patient satisfaction.It's worthy of clinical promotion.
