阿斯美联合布地奈德治疗嗜酸粒细胞性支气管炎的疗效研究

A Study on the Curative Effect of Asmeton Combined with Budesonide in the Treatment of Eosinophilic Bronchitis

目的探讨阿斯美联合布地奈德治疗嗜酸粒细胞性支气管炎(eosinophilic bmnchitis,EB)的疗效。方法选取四川大学华西医院呼吸内科门诊收治的136例EB患者为研究对象,按照随机数字表法分为试验组和对照组,每组68例。对照组患者采用超声雾化吸入布地奈德混悬液,试验组患者给予超声雾化吸入布地奈德混悬液联合阿斯美治疗,两组疗程均为10d。比较两组临床疗效及半年复发率、外周血嗜酸性粒细胞(Eos)计数、诱导痰嗜酸性粒细胞分类、呼吸一氧化氮值(FENO)、免疫球蛋白(IgE、IgA、IgG)水平、白介素5(IL-5)和白介素8(IL-8)水平。结果试验组治疗有效率(94.12%)、半年复发率(19.12%)均大于对照组治疗有效率(82.35%)、半年复发率(35.29%),差异均有统计学意义(P<0.05)。治疗后,试验组和对照组外周血Eos计数分别为(0.13±0.06)×109/L和(0.18±0.08)×109/L(P<0.05);诱导痰嗜酸性粒细胞分类分别为(3.12±1.20)%和(5.49±1.57)%(P<0.05);FENO分别为(21.36±6.52)ppb和(29.10±7.37)ppb(P<0.05);IgE分别为(138.64±14.34)g/L和(167.28±16.39)g/L(P<0.05);IgA分别为(2.74±0.58)g/L和(3.11±0.60)g/L(P<0.05);IL-5分别为(22.10±7.69)ng/L和(35.08±9.61)ng/L(P<0.05);IL-8分别为(20.09±11.20)ng/L和(29.15±13.22)ng/L(P<0.05)。结论阿斯美联合布地奈德治疗EB可提高机体免疫力,降低炎性水平,在临床治疗中有增效的作用。

Objective To investigate the curative effect of Asmeton combined with budesonide in the treatment of eosinophilic bronchitis(EB)and its effect on the immune function and short-term recurrence rate.Methods136EB cases treated in West China Hospital of Sichuan University were selected as the research objects and divided randomly into the experiment group and control group,and each group consisted of68cases.The control group was treated with ultrasonic aerosol inhalation of budesonide suspension,while the experiment group was treated with budesonide suspension combined with Asmeton.The treatment lasted for10days.The two groups were compared in terms of the clinical efficacy,half-year recurrence rate,eosinophil count in peripheral blood,classification of eosinophils in induced sputum,fractional exhaled nitric oxide(FENO),levels of immunoglobulins(IgE,IgA and IgG),interleukin5(IL-5)and interleukin8(IL-8).Results The effective rate of the experiment group(94.12%)was greater than that of the control group(82.35%)and the half-year recurrence rate of the experiment group(19.12%)was less than that of the control group(35.29%),and the differences were statistically significant(P<0.05).After treatment,the eosinophil counts in peripheral blood of the experiment and control groups were(0.13±0.06)×109/L and(0.18±0.08)×109/L;their classification levels of eosinophils in induced sputum were(3.12±1.20)%and(5.49±1.57)%;their FENO levels were(21.36±6.52)ppb and(29.10±7.37)ppb;their IgM levels were(138.64±14.34)g/L and(167.28±16.39)g/L;their levels of IgA were(2.74±0.58)g/L and(3.11±0.60)g/L;their IL-5levels were(22.10±7.69)ng/L and(35.08±9.61)ng/L;their IL-8levels were(20.09±11.20)and(29.15±13.22).The differences were statistically significant(P<0.05).Conclusion Asmeton combined with budesonide can improve the immune function and reduce the inflammatory level with clinical synergy in the treatment of patients with EB.
