NRS 2002在骨肉瘤化疗患者营养管理中的应用

The Application of NRS 2002 in Nutrition Management of Osteosarcoma Patients Undergoing Chemotherapy

目的探讨营养风险筛查(NRS 2002)在骨肉瘤化疗患者营养管理中的应用价值,为规范骨肉瘤患者营养评估、降低营养不良风险提供依据。方法选取采用AP(阿霉素+顺铂)方案进行化疗的骨肉瘤患者为研究对象。2014年4月至2015年3月采取AP方案化疗的患者38例为对照组,肿瘤患者生活质量(QOL)评分的食欲评分≤3分时给予营养干预;2015年4月至2016年3月采取AP方案化疗的患者44例为观察组,通过NRS 2002动态评估,对评分≥3分的患者给予营养干预,比较两组患者营养不良风险阳性结果率、营养不良发生率和因此终止化疗的发生率。结果两组患者筛查出存在营养风险的阳性结果率、住院期间营养不良发生率、因营养状况致化疗终止的发生率比较,差异有统计学意义(均P<0.05)。结论 NRS 2002在骨肉瘤化疗患者营养管理中的应用具有较好的敏感性,能够及时筛查出具有营养不良风险的患者,从而降低患者营养不良的发生率,提高患者对化疗的耐受性和化疗计划完成率。

Objective To explore the application value of Nutrition Risk Screening(NRS)2002 in the nutrition management of osteosarcoma patients undergoing chemotherapy and provide a basis for standardizing the nutritional assessment of osteosarcoma patients and reducing the risk of malnutrition.Methods The osteosarcoma patients undergoing chemotherapy with Adriamycin and Cisplatin were selected as the research objects.38 patients between April of 2014 and March of 2015 were selected as the control group and they were given the nutritional intervention when the scores of Appetite Scale of Quality of Life for Cancer Patients(ASQLCP)were below 3;the other 44 patients between April of 2015 and March of 2016 were selected as the observation group.They received the nutrition risk screening by NRS-2002 and were given the nutrition intervention if the ASQLCP scores were no less than 3.Results The two groups were significantly different in the positive rate of nutritional risks,incidence of malnutrition during hospitalization,and incidence of chemotherapy termination due to nutritional status(P<0.05).Conclusion NRS 2002 is effective with good sensitivity in the nutritional management of osteosarcoma patients undergoing chemotherapy by promptly screening the patients with risks of malnutrition,reducing the incidence of malnutrition and improving the tolerance of patients to chemotherapy and completion rate of their chemotherapy plan.
