脑小血管病非痴呆患者脑微出血(CMBs)数量、部位与认知功能损害的相关性分析 被引量:6

摘要 目的分析脑小血管病非痴呆患者脑微出血(CMBs)数量、部位与认知功能损害的相关性。方法随机选择2016年6月~2017年6月来我院进行脑小血管病检查的患者120例进行临床研究。根据患者MRI检查结果确定CMBs的数量和部位,对所有患者认知功能损害情况进行评价。结果 120例患者检测后有427枚CMBs,其中基底节CMBs有91枚占21.31%,其次为颞叶和额叶,分别为65枚、50枚,占15.22%、11.71%。MoCA评分≥26患者CMBs数量低于MoCA评分<26,差异有统计学意义(P<0.05)。CSVD患者MoCA总分、记忆力与CMBs数量有负相关(P<0.05)。视空间执行与额叶CMBs,语言、定向与枕叶CMBs,注意力与岛叶、DPWM、基底节CMBs,MoCA总分与DPWM部位CMBs,记忆与DPWM部位CMBs,语言与DPWM部位CMBs呈负相关(P<0.05)。结论脑小血管病非痴呆患者的脑微出血部位和数量,与认知功能损害具有密切关联。 Objective To analyze the correlation between the number and location of cerebral microhaemorrhage(CMBs)and the impairment of cognitive function in non dementia patients with small cerebral vascular disease.Methods A total of120patients with cerebral microvascular disease from June2016to June2017years from June to our hospital were randomly selected to carry out clinical study.The number and location of CMBs were determined according to the results of the patient's MRI examination,and the cognitive impairment of all patients was evaluated.Results120patients had427CMBs after detection,of which91of the basal ganglia were CMBs,accounting for21.31%,followed by temporal lobe and frontal lobe,65and50,respectively,accounting for15.22%and11.71%.The number of patients with CMBs MoCA score over26was lower than that of MoCA score of<26,there were significant differences(P<0.05).There was a negative correlation between the total score of MoCA,memory and the number of CMBs in CSVD patients(P<0.05).Visual spatial execution is negatively related to frontal lobe CMBs,language,orientation and occipital lobe CMBs.Attention was negatively correlated with the total score of insula,DPWM,basal ganglia CMBs,MoCA and DPWM location CMBs,memory and DPWM location CMBs,and language and DPWM site CMBs(CMBs)(P<0.05).Conclusion The location and number of cerebral microhaemorrhage in non dementia patients with small cerebral vascular disease are closely related to the impairment of cognitive function.
