闭合复位经皮髓内钉内固定治疗老年股骨粗隆间骨折的临床分析

Clinical observation on the treatment of intertrochanteric fractures in elderly patients with closed reduction and percutaneous intramedullary nailing

目的比较闭合复位经皮髓内钉(PFN A)和切开复位股骨近端锁定加压钢板(LC P)治疗老年股骨粗隆间骨折的临床效果。方法将46例老年股骨粗隆间骨折病例按照手术方式分为PFN A组和LC P组,对2组临床资料进行回顾性分析。结果 PFN A组手术切口长度,手术时间、术中出血量,住院天数,开始负重时间和LC P组比较差异均有统计学意义(P<0.05)。6个月后PFN A组Harris评分优良率91.7%,明显高于LC P组的68.2%(P<0.05)。结论闭合复位经皮髓内钉内固定治疗老年股骨粗隆间骨折创伤小、恢复快、疗效较好。

Objective To investigate the clinical effect of closed reduction proximal femoral nail anti-rotation(PFNA)and open reduction locking compression plate(LCP)in the treatment of intertrochanteric fractures for elderly patients.Methods46cases of elderly patients with intertrochanteric fracture were divided into PFNA group and LCP group,and the clinical data of the two groups were analyzed retrospectively.Results Between two groups,the differences of the incision length,the operation time,the amount of bleeding,the length of hospital stay,and the loading were statistically significant(P<0.05).The Harriship score in PFNA group(91.7%)was significantly higher than that of LCP group(68.2%)(P<0.05).Conclusion Closed reduction PFNA for the treatment of intertrochanteric fractures in the elderly has the advantages such as little trauma,quick recovery and good curative effect.
