子宫腺肌症与子宫肌瘤的磁共振诊断及其鉴别 被引量:4

摘要 目的分析子宫腺肌症与子宫肌瘤(肌层内型)的MRI表现及鉴别诊断。方法对19例子宫腺肌症和12例子宫肌瘤(肌层内型)患者的MRI表现对照分析,总结MRI对子宫腺肌症和子宫肌瘤(肌层内型)的诊断和鉴别诊断经验。结果本组病例的MRI检查结果与临床诊治结果一致,其中14例局限型子宫腺肌症病灶呈多位于贴近结合带的子宫肌壁外层的边缘模糊稍长T_1、稍长T_2信号,并向深层延展并穿过结合带达内膜层,内部见小点或片状高信号影,增强扫描病灶区强化程度低于周围明显强化的组织,而使病灶轮廓大致可辨;5例弥散型子宫腺肌症灶分布广泛,信号特点与局限型类似。12例子宫肌瘤(肌层内型)病灶多位于相对远离结合带的边缘清楚的类圆形的长T_1、短T_2信号,增强扫描病灶强化显著低于周围组织。全部病例的病灶均显示周围正常结构受压推移和变形。结论子宫腺肌症在MRI上病灶相对贴近结合带、呈不均匀长T_2信号、边界不清、增强扫描呈轮廓大致可见的中低度强化表现,可与子宫肌瘤(肌层内型)进行鉴别。
