早期肠内营养对大面积脑梗死患者胃肠功能及营养状况的影响

Effect of early enteral nutrition on gastrointestinal function and nutritional status in patients with large area cerebral infarction

目的:分析大面积脑梗死患者早期肠内营养(EN)对胃肠功能及营养状况的影响。方法:收集我院自2015年1月至2016年12月收治的大面积脑梗死患者86例,分为观察组(给予早期肠内营养治疗)与对照组(给予早期肠外营养治疗)两组,比较两组治疗效果。结果:观察组血清白蛋白(SAB)、三头肌皮褶厚度(TSF)、非瘫痪侧上臂三头肌肌围(MAMC)的变化幅度显著性小于对照组(P<0.05);观察组呕吐、便秘、腹胀、腹泻等胃肠道反应总发生率显著性低于对照组(P<0.05);观察组胃肠道出血、高血糖、二重感染以及肝功能损害等并发症总发生率显著性低于对照组(P<0.05)。结论:对大面积脑梗死患者采取早期肠内营养治疗,能有效改善患者的营养不良状况,提升胃肠功能,降低并发症的发生率,具有较高的推广价值。

Objective:To analyze the effect of early enteral nutrition(EN)on gastrointestinal function and nutritional status in patients with large area cerebral infarction.Methods:A total of 86 patients with large area cerebral infarction were enrolled in our hospital from January 2015 to December 2016.They were divided into observation group(given early EN therapy)and control group(given early parenteral nutrition trerapy).The therapeutic effect was compared between the two groups.Results:The rangeability of serum albumin(SAB),triceps skinfold thickness(TSF)and non-paralyzed upper arm triceps muscle(MAMC)in the observation group were significantly lower than those in the control group(P<0.0 5).The total incidence of gastrointestinal reactions such as vomiting,constipation,abdominal distention and diarrhea in the observation group was significantly lower than that in the control group(P<0.05).The total incidence of complications such as gastrointestinal bleeding,hyperglycemia,double infection and liver damage in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:For patients with large area cerebral infarction,early stage EN therapy can improve their malnutrition,improve gastrointestinal function and reduce the incidence of complications.It has high value for popularization.
