摘要
目的探讨肿瘤特异性生长因子(TSGF)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)、鳞状上皮细胞癌抗原(SCCAg)在肺癌诊断中的临床价值。方法选取2014年9月至2016年11月该院20例肺癌患者、42例良性疾病患者和54例健康体检者分别作为肺癌组、良性疾病组和健康对照组,利用雅培微粒子化学发光仪检测CYFRA21-1、SCCAg,利用罗氏电化学发光仪检测NSE,利用日本奥林巴斯5400生化分析仪检测TSGF,并对检测结果进行统计分析。结果肺癌组TSGF、NSE水平高于良性疾病组和健康对照组(P<0.05);良性疾病组CYFRA21-1水平高于健康对照组(P<0.05)。TSGF、NSE、CYFRA21-1、SCCAg联合检测肺癌的灵敏度和特异度分别为90.00%和93.00%,且曲线下面积为0.959。结论 TSGF具备广谱标记物特性,用于筛查肺癌有一定价值,且与NSE、CYFRA21-1、SCCAg联合检测可提高肺癌诊断的准确性。
Objective To investigate the clinical value of tumor specific growth factor(TSGF),neuron specific enolase(NSE),cytokeratin19fragment(CYFRA21-1),squamous cell carcinoma antigen(SCCAg)in the diagnosis of lung cancer.Methods20patients with lung cancer,42patients with lung benign diseases and54healthy people undergoing the physical examination in this hospital from September2014to November2016were served as the lung cancer group,benign diseases group and healthy control group respectively.The Abbott microparticle chemiluminescence instrument was used to detect CYFRA21-1and SCCA levels,the Roche electrochemical luminescence instrument was used to detect the NSE level and the TSGF level was detected by using the Olympus5400biochemical analyzer.Then the detection results were statistically analyzed.Results The levels of serum TSGF and NSE in the lung cancer group were higher than those in the lung benign diseases group and healthy control group,and the differences were statistically significant(P<0.05).The serum CYFRA21-1level of the lung benign disease group was higher than that in the healthy control group(P<0.05).The sensitivity and specificity of TSGF,NSE,CYFRA21-1and SCCAg combined detection were90.00%and93.00%respectively.The area under the ROC curve was0.959.Conclusion TSGF possesses wide spectrum marker characteristics,has a certain value for screening lung cancer,moreover its combined detection with NSE,CYFRA21-1and SCCAg can increase the accuracy in diagnosis of lung cancer.
