自身抗体和炎性因子在红斑狼疮患者中的诊断价值

Detection value of autoantibodies and inflammatory factors in patients with lupus erythematosus

目的探讨自身抗体、炎性因子在红斑狼疮中的临床诊断价值。方法选取2015年1月至2016年10月该院就诊的50例红斑狼疮患者作为观察组,并按红斑狼疮疾病活动指数将其进一步分为活动期亚组和非活动期亚组,同时选择健康体检女性45例作为对照组,检测各组抗核抗体(ANA)、抗dsDNA抗体、抗Sm抗体、抗核小体抗体(AnuA)、抗rRNP抗体及白细胞介素(IL)-1β、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)]水平,对结果进行统计分析。结果与对照组比较,观察组ANA、抗dsDNA抗体、抗Sm抗体、AnuA、抗rRNP抗体阳性率和血清IL-1β、IL-6、IL-8、TNF-α水平均明显升高,差异有统计学意义(P<0.05);采用5项自身抗体联合检测红斑狼疮时,观察组阳性率100.00%;与非活动期亚组比较,活动期亚组抗dsDNA抗体、AnuA阳性率明显升高,且血清IL-1β、IL-6、IL-8、TNF-α水平明显升高,差异有统计学意义(P<0.05)。结论自身抗体和炎性因子在红斑狼疮早期诊断中均有一定的价值,红斑狼疮自身抗体联合检测阳性率较高。

Objective To investigate the clinical diagnosis value of autoantibodies and inflammatory factors in lupus erythematosus.Methods50patients with lupus erythematosus in the hospital from January2015to October2016were selected as the observation group,which was further divided into the active and non active subgroups according to the disease activity index of lupus erythematosus.Meanwhile45women undergoing healthy physical examination were selected as the control group.The anti nuclear antibody(ANA),anti dsDNA antibody and anti Sm antibody,anti nucleosome antibody(AnuA),anti rRNP antibody and interleukin1beta(IL)-1,IL-6,IL-8,and tumor necrosis factor alpha(TNF-α)levels were detected.The detection results were statistically analyzed.Results Compared with the control group,the positive rates of ANA,anti-dsDNA antibody,anti Sm antibody,AnuA and anti rRNP antibody,and levels of serum IL-1β,IL-6,IL-8and TNF-αin the observation group were significantly increased,the difference was statistically significant(P<0.05).Adopting5-autoantibodies combined detection in lupus erythematosus,the positive rate in the observation group was100.00%;compared with the inactive lupus erythematosus subgroup,the positive rates of anti dsDNA antibody and AnuA in the active stage subgroup were significantly increased,moreover serum IL-1β,IL-6,IL-8and TNF-αlevels were significantly increased,the difference was statistically significant(P<0.05).Conclusion Autoantibodies and inflammatory factors have a certain value in the early diagnosis of lupus erythematosus.The sensitivity of autoantibodies combined detection is high
