血清白细胞介素-2和白细胞介素-4在糖尿病肾病早期诊断中的意义 被引量:6

摘要 目的分析血清白细胞介素-2(IL-2)和白细胞介素-4(IL-4)在糖尿病肾病(DN)早期诊断中的价值。方法选取DN患者60例(DN组)、糖尿病患者60例(DM组)和健康体检者60例(对照组),检测其血清IL-2和IL-4水平。结果各组在性别、年龄、体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)上比较,均差异无统计学意义(P>0.05)。DN组患者尿肌酐(UCr)、血肌酐(SCr)均高于其他两组,均差异有统计学意义(P<0.05)。DN组患者IL-2水平明显低于DM组和对照组,而IL-4水平却高于DM组和对照组。IL-2和IL-4单独作为标志物诊断急性肾衰竭(AKI)时,ROC曲线下面积(AUC)分别为:0.833和0.952(P<0.01),二者联合诊断时,诊断率为100%。结论 IL-2和IL-4联合检测可以相互弥补,提高DN的诊断准确率,有助于临床工作者早期发现DN,提供有效的干预措施,减少病死率。 Objective To study the serum IL-2and IL-4in the early diagnosis of diabetic nephropathy.Methods The patients were divided into two groups,patients with diabetic nephropathy(DN group)and diabetic patients(DM group)and control group,60cases respectively.ELISA method was used to detect serum IL-2and IL-4.Results Comparison of groups in gender,age,BMI,SBP,DBP,there were no significant differences(P>0.05).UCr,SCr in DN group was higher than the other two groups,the differences were statistically significant(P<0.05).The level of IL-2in patients with DN was significantly lower than that of DM patients and healthy controls,and the level of IL-4was higher than that of DM patients and healthy people.When serum IL-2and IL-4as a diagnostic marker of DN individually,the areas under the ROC curve(AUC)were:0.833and0.952(P<0.01).The diagnostic rate was100%when the combination of the two at the time of diagnosis.Conclusions The value of application of single biological markers for diagnosis of DN in clinical diagnosis is lower.However,combined with the biological markers for detection can make up for each other,and improve the diagnostic accuracy of DN,which contribute to the clinical workers in early detection of DN and provide effective intervention measures,reduce the mortality rate.
