超声乳化白内障手术联合人工晶体植入术两种不同切口对术后干眼的影响

Influence of phacoemulsification combined with intraocular lens implantation in two different incisions methods on xerophthalmia

目的探讨白内障超声乳化联合人工晶体植入术两种不同手术切口对术后干眼的影响。方法将我院进行超声乳化吸除术联合人工晶状体植入术白内障患者173例(194眼)根据手术切口方式的不同分为巩膜隧道切口组89例(103眼)和透明角膜切口组84例(91眼)。比较两组术前、术后1 d、1周、1月、3月的干眼症状评分及BUT、FL检查结果。结果随着术后时间的延长,两组患者的干眼症状评分及FL评分先升高后降低,BUT先缩短后延长,且两组患者术后1 d、1周、1月干眼症状评分及FL评分均高于术前,BUT短于术前(P<0.05);术后1 d、1周透明角膜切口组干眼症状评分及FL评分高于巩膜隧道切口组,BUT短于巩膜隧道切口组(P<0.05)。结论白内障超声乳化联合人工晶体植入术采用透明角膜切口和巩膜隧道切口均可引起患者术后出现早期干眼症状,且前者较后者引起干眼症状更加严重。

Obj ect i v e To compare the influence of phacoemulsification combined with intraocular lens implantation in two different incisions methods on xerophthalmia.M ethods A total o f 173 patients(194 eyes)with cataract who underwent phacoemulsification combined with intraocular lens implantation in our hospital were divided into scleral tunnel incision group(n=89,103 eyes)and clear corneal incision group(n=84,91 eyes).The preoperative,postoperative 1 d,1 week,1 month,3 months xerophthalmia symptom scores and BUT,F L test results were compared between the two groups.R e su l t s With the extension of postoperative time,the xerophthalmia symptom score and F L score of both groups increased firstly and then decreased,BUT firstly shortened and then lengthened,and at 1 d,1 week and 1 month after operation,the xerophthalmia symptom score and F L score were higher than those of the preoperative,the BUT were shorter than those of the preoperative(P<0.05).A t 1 d and 1 week after operation,the scores of xerophthalmia symptom and F L in the clear corneal incision group were higher than those in the scleral tunnel incision group and the BUT were shorter than those of the scleral tunnel incision group(P<0.05).C o n clu sio n Phacoemulsification combined with intraocular lens implantation with clear corneal incision and scleral tunnel incision all can cause xerophthalmia symptom in the early stage of postoperatively,and the former causes more severe xerophthalmia symptom.
