醛固酮受体拮抗剂治疗糖尿病肾病35例的疗效

Clinical effect of aldosterone receptor antagonist in the treatment of 35 cases of diabetic nephropathy

目的探讨醛固酮受体拮抗剂治疗糖尿病肾病的疗效。方法选取2015年1月至2017年6月本院收治的70例糖尿病肾病患者,随机分为对照组和观察组,各35例。对照组采用厄贝沙坦+格列齐特片(Ⅱ)治疗,观察组在对照组基础上增加螺内酯治疗。对比两组治疗前、后的肾功能指标变化,并监测两组不良反应发生情况。结果治疗后,两组ALB、ALD、SCr、24 h尿蛋白定量水平显著降低,GFR水平显著升高,且观察组优于对照组(P<0.05)。观察组不良反应总发生率(8.57%)略高于对照组(5.71%),但差异不显著(P>0.05)。结论糖尿病肾病采用醛固酮受体拮抗剂治疗效果可靠,可有效控制患者肾病进展。

Objective To investigate the efficacy of aldosterone receptor antagonist in the treatment of patients with diabetic nephropathy.Methods From January 2015 to June 2017,70 patients with diabetic nephropathy in our hospital were randomly divided into control group and observation group,with 35 cases in each group.The control group was treated with irbesartan+gliclazide tablets(II)treatment.The observation group was treated with spironolactone on the basis of the control group.The changes of renal function indexes were compared between the two groups before and after treatment,and the adverse reactions were monitored.Results After treatment,the ALB,ALD,SCr and 24 h urinary protein level in the two groups significantly decreased,and GFR level significantly increased,and those in the observation group were better than the control group(P<0.05).The total incidence of adverse reactions in the observation group(8.57%)was slighty higher than that in the control group(5.71%),but the difference was not significant(P>0.05).Conclusion Aldosterone receptor antagonist in the treatment of diabetic nephropathy has reliable treatment effect,which can effectively control the progress of nephropathy.
