超声多普勒检查颈动脉斑块硬化情况与冠心病的相关性

Correlation between carotid atherosclerotic plaque and coronary heart disease by Doppler ultrasonography

目的对超声多普勒检查颈动脉斑块硬化情况与冠心病的相关性进行分析探讨。方法选择2015年1月至2016年12月我院收治的96例经冠状动脉造影检查确诊为冠心病患者,根据病变程度将其分成单支病变组、双支病变组和三支病变组,每组32例,另取同期健康体检者32例作为对照组,对所有受试者展开双侧颈动脉多普勒超声检查,并对检查结果进行统计分析。结果冠心病患者的颈动脉超声参数β、Ep、AC、PWVβ值均高于对照组(P<0.05),并且随着冠脉病变程度的增加而增加(P<0.05);冠心病患者颈动脉内膜厚度高于对照组(P<0.05),随着病变程度增加,颈动脉内膜厚度逐渐升高(P<0.05);冠心病患者不同病变支数者不稳定斑块检出数存在明显差异(P<0.05);冠心病组斑块积分均明显低于对照组(P<0.05)。结论经多普勒超声对冠状动脉硬化斑块进行检查,可为临床冠心病的早期诊断和病情评估提供可靠的参考依据,值得关注。

Objective To analyze and explore the correlation between carotid atherosclerotic plaque and coronary heart disease by Doppler ultrasonography.Methods From January 2015 to December 2016,96 cases with coronary heart disease diagnosed by coronary angiography were enrolled and divided into single vessel disease group,double vessel disease group and triple vessel disease group according to the degree of lesion,with 32 patients in each group.Thirty-two healthy subjects were taken as the control group.All subjects underwent bilateral carotid artery Doppler ultrasonography,and the results were analyzed statistically.Results The values of carotid ultrasound parameters茁,Ep,AC,PWV茁in patients with coronary heart disease were higher than those in the control group(P<0.05),and those increased with the increase of coronary artery lesion(P<0.05).The carotid artery intima thickness in patients with coronary heart disease was higher than that of the control group(P<0.05).With the increase of the lesion,the carotid artery intima thickness increased gradually(P<0.05).The number of unstable plaque detected in different degree of lesion with coronary heart disease was significantly different(P<0.05).The plaque score of the coronary heart disease group were significantly lower than that of the control group(P<0.05).Conclusion The examination of coronary atherosclerotic plaques by Doppler ultrasound can provide a reliable reference for early diagnosis and assessment of coronary heart disease,which is worthy of attention.
