构建三环管理模式提升住院医师规范化培训质量

Establishing Three-phases Management strategy to improve the qualities of standardized residency training

本研究介绍了中山大学附属第一医院在28年住院医师规范化培训经验基础上,总结形成的培训过程管理-"三环管理"模式,详细阐述了初始环节、中间环节和终末环节的工作内容和要点。初始环节指住院医师报到一个月内需完成的工作,包括:开展岗前培训、组织临床能力测评、确定培训年限、制定个性化培训计划、确定全程导师和建立个人档案等。中间环节指从住院医师进入第一个科室培训到完成全部轮转计划的过程,是培训工作的核心部分。要求医院层面、专业基地、各轮转专科和带教老师"四位一体",有效协作,围绕临床胜任力,建立完善的培训和考评体系,为住院医师提供富有实效的临床培训和学习检验标准。终末环节要求严格做好住院医师结业考核资格审核、考务安排以及最终培训合格证书的审核发放。经过多年的探索和实践,本院"三环管理"模式已成熟,有效促进各方协作,显著提升培训质量,并产生显著的区域辐射效应,在广东省得到广泛推广。

This study introduces the Three-phases Management strategy created by The First Affiliated Hospital of Sun Yat-sen University,based on 28 years’experiences of standardized residency training.Three-phases Management includes initial phase,intermediate phase and terminal phase.Each phase includes special work.Initial phase contains of pre-training education,organizing examinations of clinical skills,deciding the period of training,making personalized training plans,identifying the mentor,establishing files,etc.Intermediate phase includes strict rotations and evaluations,which is the core of training program.In this phase,it requires the hospital administration,training base,departments of rotations and teachers to cooperate effectively,in order to build consummate training and evaluation systems based on clinical competence,which provides the residents a practical standard for clinical training and studying evaluations.Terminal phase comprises of strictly examining and verifying the qualifications for stage examinations,making arrangements of exams and issuing training certificates.After years of exploration and practice,our Three-phases Management strategy has become mature,which effectively promoted the collaboration,significantly improved the qualities of residency training,and had been widely applied in institutions throughout Guangdong.
