个案管理模式在降低心力衰竭患者再入院率与提高治疗依从性中的应用

Application of case management model in reducing heart failure patients' readmission rate and improving treatment compliance

目的:探讨个案管理模式在降低心力衰竭患者再入院率与提高治疗依从性中的应用效果。方法:选择2015年10月至2017年8月我科收治的心力衰竭患者86例,将患者随机等分为研究组和对照组,对照组给予常规护理模式,研究组给予个案管理模式。比较两组患者的自我管理情况、出院6个月之内的再入院率以及治疗依从率。结果:研究组各项自我管理评分均高于对照组(P<0.05),出院6个月内再入院率低于对照组(P<0.05),患者的治疗依从度高于对照组(P<0.05)。结论:个案管理模式能够有效降低心力衰竭患者的再入院率,提高患者治疗依从性,提升临床综合治疗效果,是临床实践中的理想方式之一。

Objective:To explore the application effect of case management model in reducing heart failure patients’readmission rate and improving treatment compliance.Methods:To select a total of 86 patients with heart failure treated in our department from October 2015 to August 2017 as the research objects and the patients were randomly divided into the study group and the control group.The control group was given regular nursing mode.The study group was given the case management model.The self-management status of the two groups of patients,the rate of re-admission within 6 months of discharge,and the compliance rate of treatment were compared.Results:The self-management scores of the study group were higher than those of the control group(P<0.05).The rate of re-admission was lower than that of the control group within 6 months of discharge(P<0.05).The patient's treatment compliance was higher than that of the control group(P<0.05).Conclusion:The case management model can effectively reduce the rate of re-admission in patients with heart failure,improve patient compliance,and the effectiveness of clinical comprehensive treatment.It is one of the ideal methods in clinical practice.
