大骨瓣减压手术在治疗重型颅脑创伤的临床意义分析

Clinical Significance of Large Bone Flap Decompression in the Treatment of Severe Craniocerebral Trauma

目的研究大骨瓣减压手术治疗重型颅脑创伤的临床疗效。方法选取我院收治的重型颅脑创伤患者94例,随机分为对照组、观察组,各47例,对照组采用常规手术治疗,观察组采用大骨瓣减压手术治疗,对比两组临床疗效。结果经GCS评估后,对照组死亡4例,植物状态11例,严重残疾19例,中度残疾10(21.28%)例,恢复良好3(6.38%)例;观察组死亡2例,植物状态8例,严重残疾10例,中度残疾18(38.30%)例,恢复良好9(19.15%)例。观察组预后良好率为57.45%(27/47)显著高于对照组27.66%(13/47)(χ~2=8.529,P=0.004<0.05),差异有统计学意义。结论应用大骨瓣减压手术治疗重型颅脑创伤可有效降低颅内压,预后效果良好,值得推广。

Objective To study the clinical effect of large bone flap decompression in the treatment of severe craniocerebral trauma.Methods 94 cases of severe craniocerebral trauma in our hospital were selected and randomly divided into control group and observation group,47 cases in each group.The control group was treated by routine operation,while the observation group was treated with large bone flap decompression operation.The clinical efficacy of two groups was compared.Results After GCS assessment,4 cases of the control group died,vegetative state in 11 cases,severe disability in 19 cases,moderate disability in 10 cases(21.28%),good recovery in 3(6.38%)cases;In the observation group,2 cases died,8 cases of vegetative state,severe disability in 10 cases,moderate disability in 18 cases(38.30%),recovery good 9(19.15%)cases.The good prognosis of the observation group was 57.45%(27/47),which was significantly higher than that of the control group 27.66%(13/47),χ2=8.529,P=0.004<0.05),and the difference was statistically significant.Conclusion The application of large bone flap decompression in the treatment of severe craniocerebral trauma can effectively reduce the intracranial pressure,and the prognosis is good.It is worth of popularizing.
