脾脏血管瘤的超声造影表现及鉴别诊断

Study on the Diagnosis and Differential Diagnosis of Contrastenhanced Ultrasound for Splenic Hemangioma

目的探讨脾脏血管瘤的超声造影(CEUS)表现及鉴别诊断要点。方法回顾性分析经增强CT、MRI检查临床确诊或手术病理证实的15例30个病灶脾脏血管瘤的CEUS资料。结果 15例30个病灶中,28个脾脏血管瘤病灶常规超声均为高回声病灶,2个病灶为低回声病灶,经超声造影后15例30个病灶均在动脉期出现增强,其中8个病灶表现为动脉期呈周边结节状向心增强,静脉期高于或等于脾脏回声,15个病灶表现为动脉期均匀强化,静脉期高于或等于脾脏回声,7个病灶表现为动脉期低增强,静脉期逐步向内缓慢充填后呈低增强。结论 CEUS能动态显示脾脏血管瘤的增强特征,有助于其诊断和鉴别诊断。

Objective To explore the diagnosis and differential diagnosis of splenic hemangioma(CEUS).Methods The CEUS data of 15 cases with 30 lesions of the spleen were retrospectively analyzed,which were confirmed by enhanced CT and MRI examination or confirmed by surgical pathology.Results Among 15 cases with 30 lesions,28 splenic hemangiomas were hyperechoic lesions,2 lesions were hypoechoic lesions under conventional ultrasound.15 cases with 30 lesions after contrast-enhanced ultrasound appeared in arterial enhancement,including 8 lesions in arterial phase showed peripheral nodular centripetal enhancement with equal enhancement in venous phase,15 lesions showed homogeneous enhancement on arterial phase with higher than or equal to the spleen echo in venous phase,7 lesions showed low enhancement in arterial phase with gradually inward filling to hypo enhancement in venous phase.Conclusion CEUS can dynamically display the enhancement characteristics of the spleen hemangioma,which is helpful for the diagnosis and differential diagnosis.
