腹腔镜肝叶切除术治疗肝内胆管结石的临床疗效及安全性

Clinical Efficacy and Safety of Laparoscopic Hepatectomy in the Treatment of Intrahepatic Bile Duct Stones

目的探讨腹腔镜肝叶切除术治疗肝内胆管结石的临床疗效及安全性。方法纳入2015年4月—2016年11月90例肝内胆管结石患者以数字表法分组。对照组采用传统的开腹手术治疗,观察组腹腔镜肝叶切除术治疗。比较两组肝内胆管结石治疗效果;手术和手术后康复相关指标;干预前后患者生存质量评分;并发症情况。结果观察组肝内胆管结石治疗效果高于对照组,P<0.05;观察组手术和手术后康复相关指标优于对照组,P<0.05;干预前两组生存质量评分相近,P>0.05;干预后观察组生存质量评分优于对照组,P<0.05。观察组并发症更少,P<0.05。结论腹腔镜肝叶切除术治疗肝内胆管结石的临床疗效及安全性高,可减轻创伤,维持机体稳态,减少并发症,改善生存质量。

Objective To investigate the clinical efficacy and safety of laparoscopic hepatectomy in the treatment of intrahepatic bile duct stones.Methods From April 2015 to November 2016,90 cases of intrahepatic bile duct stones were grouped by digital table method.The control group was treated with traditional laparotomy.The observation group was treated by laparoscopic hepatectomy.To compare the effect of two groups of intrahepatic bile duct stones,the related indexes of operation and postoperative rehabilitation,the quality of life of the patients before and after intervention,and the complications.Results The effect of intrahepatic bile duct stones in the observation group was higher than that of the control group,P<0.05.The related indexes in the observation group were better than those in the control group,P<0.05.The score of quality of life of the two groups was similar before intervention,P>0.05.The score of quality of life in the observation group was better than that of the control group,P<0.05.There were fewer complications in the observation group,P<0.05.Conclusion Laparoscopic hepatectomy is effective and safe for the treatment of hepatolithiasis,which can reduce trauma,maintain homeostasis,reduce complications and improve quality of life.
