细胞DNA倍体分析法在良恶性胸腹水鉴别中的应用价值 被引量:5

摘要 目的探讨DNA倍体分析方法在良恶性胸腹水中临床应用价值。方法收集274例患者胸腹水同时进行DNA倍体分析法、常规脱落细胞分析法、染色体分析法三种检查,阳性及可疑阳性病例通过病理诊断进一步证实。结果 DNA倍体分析法、常规脱落细胞学分析法、染色体分析法阳性率分别为23.36%、27.73%、40.88%。DNA倍体分析法与细胞分析法比较,差异无统计学意义(P>0.05);DNA倍体分析法与染色体分析法比较,差异有统计学意义(P<0.05)。染色体分析法准确率较高,达97.44%。DNA倍体分析法特异性为79.20%,敏感性为57.14%;细胞学分析法特异性为83.58%;敏感性为67.86%。结论DNA倍体分析可作为胸腹水良恶性分析的重要指标,其阳性结果可为以作为临床诊疗依据。 Objective To investigate the diagnostic value of DNA analysis withDNA image cytometry in Diagnostic Benign and malignant pleuroperitoneal fluids.Methods Thepleuroperitoneal fluids cell samples were collected from 274 patients participating in the comparison by the DNA ploidy analysis,conventional cytology examination and chromosome analysis all the time.Andthepositive and suspected positive cases were further confirmed by pathological diagnosis.Results The positive rate of DNA ploidy analysis,conventional cytology examination and chromosome analysis were 23.36%,27.73%and 40.88%respectively.To compare the DNA ploidy analysis withconventiona cytology examination,therewas no significant difference(P>0.05).And to compare the DNA ploidy analysis with chromosome analysis,the difference was statistically significant(P<0.05).The accuracy rate ofthe chromosome analysis is highest and up to 97.44%.The specificity and sensitivity of DNA ploidy analysis is 79.20%and 57.14%.And The specificity and sensitivity of conventional cytology examinationis 83.58%and 67.86%.Conclusion DNA ploidy analysis may be used as an important reference index of benign and malignant pleuroperitoneal,and the positive results may be provided of the value for clinical diagnosis.
