不同程度慢阻肺稳定期患者血栓前状态的临床研究

Clinical analysis of different degrees of stable chronic obstructive pulmonary disease patients with prethrombotic state

目的研究并分析不同程度慢性阻塞性肺疾病(慢阻肺)稳定期患者血栓前状态的临床特征。方法选取2015年4月-2016年8月在我院治疗后复查的慢阻肺稳定期患者60例为观察组,另选同期健康体检者60例为对照组。同时,将观察组患者根据病情程度的不同分为轻度组、中度组、重度组,比较不同组别间的凝血指标水平,并进行相关性分析。结果观察组HB、D-D、FIB水平分别为(144.8±18.7)g/L、(512.4±322.5)μg/mL、(3.6±0.8)g/h,均显著高于对照组的(134.8±13.2)g/L、(140.3±84.7)μg/mL、(1.9±0.5)g/h。轻度组患者HB、D-D、FIB水平分别为(139.4±14.5)g/L、(188.4±91.3)μg/mL、(2.6±0.7)g/h,而中度组又显著低于重度组,差异均有统计学意义(均P<0.05)。经Spearman相关性分析可得,慢阻肺稳定期病情程度与HB、D-D、FIB水平均呈显著正相关关系(r=0.573,P=0.000;r=0.831,P=0.000;r=0.767,P=0.000)。结论不同程度稳定期慢阻肺患者血栓前状态具有一定差异,病情程度越严重血栓前状态便越明显。

Objective To study the clinical characteristics of different degrees of COPD patients with plateau prethrombotic state.Methods 60 re-examined stable COPD patients were selected from April 2015 to August 2016 in our hospital as the observation group,and another 60 healthy physical examination people were taken as the control group.At the same time,according to the disease degree the observation group were divided into the mild group,the medium group,and the severe group.The coagulation indexes were compared,and correlation analysis was carried out.Results The levels of HB,D-D and FIB were(144.8±18.7)g/L,(512.4±322.5)μg/mL,and(3.6±0.8)g/h respectively in the observation group,which were significantly higher than those of the control group[(134.8±13.2)g/L,(140.3±84.7)μg/mL,(1.9±0.5)g/h].The level of HB,D-D and FIB were significantly lower in the mild group than in the moderate group and the severe group,and the moderate group was significantly lower than the severe group.The Spearman correlation analysis showed that the degree of COPD stabilization condition and the level of HB,D-D and FIB had significantly positive correlation(r=0.573,P=0.000;r=0.831,P=0.000;r=0.767,P=0.000).Conclusion Different degree of stabilization thrombosis in COPD patients has certain difference,and the more severe the disease is,the more obvious prethrombotic state.
