基于国际糖尿病足工作组风险分级系统评估增加糖尿病足风险的影响因素

Analyzing the Diabetic Foot Influencing Factors According to IWGDF Risk Stratification

目的通过国际糖尿病足工作组(International World Group ofDiabeticFoot,IWGDF)风险分级系统评估增加糖尿病足风险的影响因素。方法 2016年9月至2017年1月,便利抽样法选取广州市某三级甲等综合性医院内分泌科住院的糖尿病患者为研究对象。自行设计问卷,对糖尿病患者的社会人口学资料、疾病相关资料、足部筛查资料进收集,并根据IWGDF糖尿病足风险分级系统对患者的足部情况进行分级。结果患者的年龄、糖尿病病程、糖化血红蛋白水平以及是否有高血压病、周围神经病变、外周血管病变、脑卒中病史均是增加糖尿病足风险的影响因素(均P<0.05)。结论高龄、糖尿病病程长、血糖控制不佳、具有糖尿病并发症、合并高血压病、脑卒中的患者更易发生糖尿病足,临床工作人员应早期识别此类患者,并进行有效管理,预防其糖尿病足的发生。

Objective To analyze the diabetic foot risk factors according to international world group of diabetic foot(IWGDF)risk stratification.Methods By convenience sampling,diabetic patients from September 2016 to January 2017 in a general hospital in Guangzhou were selected and by the social demographic data,disease related data and foot screening data were collected.The patient’s foot condition was rated according to IWGDF risk stratification.Results Patients’age,diabete length,level of glycosylated hemoglobin A1c and hypertension,diabetic peripheral neuropathy,diabetic peripheral vasculopathy and stroke history were the influencing factors of increasing the risk of diabetic foot(all P<0.05).Conclusion Diabetes patients with those factors including advanced age,longer length of diabetes,poor glycemic control,diabetes complications,combined with hypertension and a history of stroke were more likely to develop diabetic foot.Clinical medical staff should identify them early and manage effectively to prevent the occurrence of diabetic foot.
