齐刺电针联合前列地尔注射液治疗暴聋的临床观察

Clinical Observation of Triple-needling of Electro-acupuncture Combined with Alprostadil Injection in the Treatment of Sudden Deafness

目的:观察齐刺电针法联合前列地尔注射液治疗暴聋的临床疗效。方法:将90例患者随机分为3组,每组各30例(单耳:30耳),A组采用齐刺电针(穴取耳门、听宫、听会)联合前列地尔注射液静脉注射,B组采用穴位注射(翳风透翳明)联合前列地尔注射液静脉注射,C组仅采用前列地尔注射液静脉注射治疗,分别于治疗的1周、2周和治疗结束后4周进行疗效对比和观察。结果:A组治疗后总有效率达86.7%;B组总有效率70%,C组为56.7%,3组比较P<0.05。治疗后3组纯音听阈测定积分均降低且组内差异有统计学意义(P<0.01);A组与B组比较14天和随访期有统计学意义(P<0.05);A组与C组比较各时间点均有统计学意义(P<0.05);B组与C组比较7天有统计学意义(P<0.05)。3组ABRⅠ波Ⅲ波Ⅴ波疗前与疗后比较有统计学意义;A组与B组治疗后与随访期比较无统计学意义(P>0.05);A组与C组各时间点比较均有统计学意义(P<0.05);B组与C组治疗后与随访期比较无统计学意义(P>0.05)。结论:电针联合前列地尔注射液治疗气滞血瘀型暴聋疗效显著,为中西医结合治疗暴聋提供了一定的临床经验。

Objective:To observe the clinical efficacy of triple-needling of electro-acupuncture combined with Alprostadil injection in the treatment of sudden deafness.Methods:90 patients were randomly divided into three groups,with 30 cases in each group(single ear:30 ears);group A was treated with triple-needling of electro-acupuncture(points of TE21,SI19,and GB2 were chosen)combined with Alprostadil injection,group B was treated with point injection(TE17-to-EX-HN14)combined with Alprostadil injection,group C was only treated with Alprostadil injection.Clinical effect were observed and compared respectively 1 w,2 w of the treatment and 4w after the treatment.Results:The total effective rate was 86.7%in group A,70%in group B,and 56.7%in group C;the difference was statistically significant among the three groups(P<0.05).Scores of pure tone audiometry were all significantly decreased in the three groups after the treatment,and there was difference between pre-treatment and post-treatment in the same group(P<0.01);it was different between group A and group B on 2w of the treatment and on 4w after the treatment(P<0.05);it was different between group A and group C at all time point(P<0.05);there was no difference between group A and group B after the treatment and during the follow-up(P>0.05).Conclusion:The therapy of electro-acupuncture combined with Alprostadil injection has significant effect on sudden deafness of the pattern of qi stagnation and blood stasis.It provides reference for the combination treatment of Chinese and western medicine for this kind of disease.
