摘要
目的:观察针刀松解术对类风湿关节炎肘关节僵硬患者血清中MDA与SOD的变化,探讨针刀松解术的作用机制。方法:将60例类风湿肘关节僵硬患者分为基础治疗组、针刺组与针刀组,每组各20例。所有患者均采用中西医基础治疗方案治疗,基础治疗组不针对肘关节局部进行治疗,针刺组采用针刺法治疗,针刀组采用针刀松解术治疗。治疗结束后,抽取患者肘静脉血,检测血清中MDA与SOD,ESR、CRP及RF含量的变化。结果:经过治疗后,各组MDA水平呈下降趋势,SOD水平呈上升趋势。与治疗前比较,差异有显著性意义(P<0.05);与基础治疗组比较,针刀组MDA水平下降趋势与SOD水平上升趋势更为明显,差异有显著性意义(P<0.05);与针刺组比较,针刀松解治疗后,SOD水平上升趋势更为明显,差异有显著性意义(P<0.05)。与治疗前比较,针刺组与针刀组中ESR、CRP及RF含量降低,经统计学处理,差异有显著性意义(P<0.05);与基础治疗组比较,针刺组与针刀组中ESR、CRP及RF含量降低,差异有显著性意义(P<0.05);与针刺组比较,针刀组ESR、CRP及RF含量降低更明显,差异均有统计学意义(P<0.05)。结论:针刀松解术治疗能增加血清自由基清除剂超氧化物歧化酶(SOD)含量,有效清除多余自由基,从而治疗类风湿肘关节僵硬。
Objective:To observe the serum changes of MDA and SOD in rheumatoid arthritis(RA)patients with elbow joint stiffness by acupotomy,and to explore the mechanism of acupotomy.Methods:60 RA patients were divided into the basic treatment group,the acupuncture group,and the acupotome group,with 20 cases in each group.All patients were treated with the basic treatment of traditional Chinese and western medicine.The basic treatment group was not given local treatment of the elbow joint.The acupuncture group and the acupotomy group were also treated with acupuncture and acupotomy correspondingly.At the end of the treatment,the elbow venous blood was collected and the changes of serum MDA,SOD,ESR,CRP and RF were detected.Results:After the treatment,MDA level of each group showed a downward trend and SOD level showed an upward trend,the difference was significant compared to those before the treatment(P<0.05);the changes of the acupotomy group was more significant compared to those of the basic treatment group and the acupuncture group(P<0.05).In terms of the levels of ESR,CRP and RF,they were significantly decreased in the acupuncture group and the acupotome group after the treatment(P<0.05);of which there were significant differences in the acupuncture group and the acupotome group,compared to the basic treatment group(P<0.05);the decreasing level was more significant in the acupotome group than that in the acupuncture group(P<0.05).Conclusion:Acupotomy treatment can increase SOD content,effectively remove excess free radicals,thus to relieve stiffness of RA.
