吻合器痔上黏膜环形切除术治疗嵌顿痔的临床研究

The clinic research of prolapse and hemorrhoids methods in the treatment of incarceration of internal hemorrhoids

目的考察以吻合器痔上黏膜环切术(procedure for prolapse and hemorrhoids,PPH)对嵌顿痔进行治疗的临床效果,评估其在临床应用的价值。方法回顾性分析本院2014年1月至2015年12月收治的嵌顿痔患者共计79例,根据患者的临床状况及个人要求,将上述病例分成两组,分别为成PPH(治疗组)组41例,和MMH(对照组)组38例。对比两组在治疗过程中的手术情况、住院情况、术后恢复情况及并发症,复发率发生情况等指标之间的差别。以对比两种方法的治疗效果。结果对于纳入本研究所考察的手术时间、住院时间、术后疼痛指数、恢复工作时间等指标之间对比可以看出,PPH组明显优于MMH组(P<0.05);并发症,复发率发生情况等情况对比,并无显著性差异(P>0.05);另,PPH组的住院费用高于MMH组。结论临床可以用PPH术对嵌顿痔进行治疗,该方法手术效果好,患者恢复快。对于纳入研究的短期结果看,其效果优于MMH术,但长期疗效尚需进一步随访确认。

Objective:The present research investigated the clinic effect of procedure for prolapse and hemorrhoids(PPH)in the treatment of annular stapler hemorrhoidal mucosa resection.Methods:The incarcerated patients,as 79 cases were received and cured in our hospital form January 2014-January 2015,were randomly separated into two groups,the PPH group(experimental group)enrolled 41 cases who were operated with PPH,and 38 cases were regard as the MMH group(control group)who were operated with Milligan surgery.To evaluate the curative effect of PPH and MMH in the treatment of annular stapler hemorrhoidal mucosa resection,operative condition,hospital condition,postoperative recovery and postoperative complications,recurrence were consider as the investigate factors.Result:Compare with the control group,experimental group obtained a higher symptoms improved satisfaction rate,shorter operative time,shorter length of hospital stay and a lower postoperative pain score,PPH group obtained a significant higher mark compare with MMH group(P<0.05);On the other side,regard incidence of postoperative complications and recurrence rate,there was no significant different between the control group and experimental group(P>0.05).in addition,the PPH exhibited a higher hospital fee compare with MMH group.Conclusion:As a feasible treatment of incarcerated hemorrhoid,PPH exhibited less pain,shorter hospitalization time and the advantages of rapid recovery,but the long-term efficacy still need to follow-up.
