不同手术治疗后喉癌患者气管感染、肺功能损伤发生情况及其对肿瘤复发的影响

Tracheal Infection and Pulmonary Function Injury in Patients with Laryngeal Carcinoma after Different Surgical Therapies and Its Effect on Tumor Recurrence

目的探讨不同手术治疗后喉癌患者气管感染及肺功能损伤发生情况,分析其对肿瘤复发的影响。方法选取2011年6月—2016年12月收治的80例喉癌,根据手术方案分为观察组和对照组,每组40例。对照组采用喉全切术,观察组采用喉部分切除术。比较两组术后气管感染、肺功能损伤及肿瘤复发情况,并分析肿瘤复发的影响因素。结果观察组术后早期生活质量优于对照组,气管感染率低于对照组(P<0.05)。观察组术后1 s用力呼气容积(FEV1%)升高和25%肺活量最大呼气流量(V25)降低程度均优于对照组(P<0.05)。本组肿瘤复发率为27.50%。年龄、淋巴转移、TNM分期、术后生活质量评分和手术方式为肿瘤复发的影响因素(P>0.05)。结论喉癌应根据患者情况制定合适的手术方式,以提高生存率及术后生存质量。

Objective To investigate incidence of tracheal infection and pulmonary function injury in patients with laryngeal carcinoma after different surgical therapies and its effect on tumor recurrence.Methods A total of 80 patients with laryngeal carcinoma admitted during June 2011 and December 2016 were divided into observation group(n=40)and control group(n=40)according to different surgical treatments.Control group was treated with total resection,while observation group was treated with partial laryngectomy.In two groups,conditions of tracheal infection,pulmonary function injury and tumor recurrence were compared,and influencing factors of tumor recurrence were analyzed.Results In observation group,quality of life in early after surgery was significantly better(P<0.05),incidence rate of tracheal infection was significantly lower than those in control group(P<0.05);increased level of forced expiratory volume in 1 second(FEV1%)and decreased levels of maximal expiratory flow in 25%vital capacit(Vmax25)were significantly higher than those in control group(P<0.05).Rate of tumor recurrence was 27.50%in this study,and age,lymphatic metastasis,tumour node metastases(TNM)stage,postoperative score of life quality and surgical method were influencing factors of tumor recurrence(P>0.05).Conclusion Laryngeal carcinoma patients should give appropriate surgical methods according to patient's condition to improve postoperative quality of life and survival rate.
