摘要
目的探讨子痫前期的主要发病机制。方法选取2013年8月—2017年6月沧州市中心医院产科诊断的妊娠期高血压、子痫前期、重度子痫前期患者各40例,并选择同期正常妊娠孕妇40例作为对照组,分析4组相关血清因子水平。结果对照组、妊娠期高血压组、子痫前期组和重度子痫前期组中血清对氧磷酶1和脂联素、高密度脂蛋白胆固醇水平逐渐降低,脂蛋白相关磷脂酶A2、白介素-6、肿瘤坏死因子-α、C反应蛋白、总胆固醇、甘油三酯和低密度脂蛋白胆固醇逐渐升高(P<0.05)。结论氧化应激、炎性反应以及血管内皮损伤均可能为子痫前期的发生及进展的相关机制,除控制血压外,合理适当的抗感染及保护血管等也为治疗子痫前期的关键措施。
Objective To investigate main pathogenesis of preeclampsia patients.Methods A total of 40 pregnant women with gestational hypertension,40 pregnant women with preeclampsia and 40 pregnant women with severe preeclampsia during August 2013 and June 2017 were recruited in this study,and other 40 normal pregnant women were selected as control group,and related levels of serum factors were analyzed in four groups.Results In control,gestational hypertension,preeclampsia and severe preeclampsia groups,serum paraoxonase-1(PON-1)and adiponectin(APN)and high density lipoprotein cholestorol(HDL-L)levels were gradually decreased,while lipoprotein related phospholipase A2(Lp-PLA2),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP),total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholestorol(LDL-C)levels were gradually increased(P<0.05).Conclusion Oxidative stress,inflammatory reaction and vascular endothelial injury may be all related to pathogenesy and progression of preeclampsia,and treatments such as appropriate anti-infection and protecting blood vessels are also key measures in treatment of preeclampsia besides of controlling blood pressure.
