延续性护理模式在医养结合模式下家庭护理中的应用

Application of Continuous Nursing Mode in Family Nursing under the Combination of Medical Care and Nursing

目的:探讨延续性护理干预模式在医养结合模式下家庭护理中的应用价值。方法:选取2015年6月~2016年12月于某院康复科内实施医养结合养老模式的老人80例作为观察对象,采用随机数字表法将其分为观察组与对照组,对照组40例老人接受常规家庭护理,观察组40例则接受延续性护理干预模式,比较两组老人生活质量、精神状态等临床指标。结果:两组护理前日常生活能力量表(ADL)、老人生命质量测评量表(QOL-AD)、简易精神状态检查表(MMSE)评分比较无显著差异(P>0.05),干预后观察组老人ADL、QOL-AD、MMSE评分明显优于对照组(P<0.05);干预过程中观察组跌倒、坠床、感染等发生率明显低于对照组(P<0.05)。结论:在老人社区医养结合家庭护理中采取延续性护理干预可有效改善老人生活质量及精神状态,具有临床应用及推广价值。

Objective:To investigate the application value of continuous nursing intervention mode in family nursing under the combination of medical care and nursing.Methods:80 elderly patients with combination of medical care treated in a hospital from June 2015 to December 2016 were selected as observation objects and randomly divided into observation group and control group by random digital table method,with 40 cases in each group.The control group received routine nursing home care,and the observation group received continuous nursing intervention model,then the quality of life,mental status and other clinical indexes between the two groups were compared.Results:There was no significant difference in the scores of ADL,QOL-ADN and MMSE between the two groups before nursing(P>0.05).After intervention,the scores of ADL,QOL-AD,MMSE in the observation group were significantly better than those in the control group(P<0.05).The incidence of falling,falling bed and infection in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:Continuous nursing intervention in elderly community health care combined with family nursing can effectively improve the quality of life and mental state of the elderly,and has clinical application and popularization value.
