TOPOⅡα、Ki-67及p53在三阴性乳腺癌中的表达及临床意义

Expression and clinical significance of TOPO Ⅱ α, Ki-67 and p53 in triple negative breast cancer

目的:探讨TOPOⅡα、Ki-67及p53在三阴性乳腺癌组织中的表达及临床意义。方法:免疫组织化学法对265例浸润性乳腺癌患者肿瘤组织中的TOPOⅡα、Ki-67及p53蛋白表达水平进行检测,并分析其与肿瘤大小、TNM分期、淋巴结转移、组织学分级之间的关系。结果:265例浸润性乳腺癌患者中,三阴性乳腺癌患者占15.47%(41例),非三阴性乳腺癌占84.53%(224例)。三阴性乳腺癌中TOPOⅡα、Ki-67及p53的高表达率分别为78.05%、73.17%、65.85%,与非三阴性组相比差异有统计学意义(P<0.05)。TOPOⅡα的高表达与肿瘤直径、组织分级相关(P<0.05);p53的高表达与组织分级、淋巴结转移情况相关(P<0.05);Ki-67的高表达与组织分级和肿瘤直径相关(P<0.05)。TOPOⅡα与Ki-67及p53在三阴性乳腺癌中的表达具有一定的相关性。结论:通过检测三阴性乳腺癌中TOPOⅡα、Ki-67及p53的表达,有助于临床医师对三阴性乳腺癌的生物学行为进行更好的评估,也为更精准化疗方案的选择提供更多依据。

Objective:To detect the expression and clinical significance of TOPOⅡα、Ki-67 and p53 in triple negative breast cancer(TNBC).Methods:The expression levels of TOPOⅡα、Ki-67 and p53 were detected by IHC in 265 cases with invasive breast cancer.And analyze the relationship between the expressions and clinicopathological features including tumor size,TNM stage,lymph node metastasis and histological grade.Results:Among 265 cases of breast cancer patients,TNBC patients accounted for 15.47%(n=41),non-TNBC accounted for 84.53%(n=224).the high expression rate of TOPO IIα,Ki-67 and p53 in TNBC were 78.05%,73.17%,and 65.85%,respectively,the difference was statistically significant compared with non-TNBC group(P<0.05).The high expression of TOPO IIαwas correlated with tumor volume,histological grading(P<0.05);and the high expression of p53 was associated with histological grade,lymph node metastasis(P<0.05);the high expression of Ki-67 was related with histological grade and tumor volume(P<0.05).The expression of TOPO IIα,Ki-67 and p53 in TNBC was correlated.Conclusion:By detecting the expression of TOPOⅡα,Ki-67 and p53,clinicians have a better understand of the biological behavior of TNBC,and provide an important basis for the selection of chemotherapy regimens.
