掌击手法复位石膏外固定治疗儿童内收型孟氏骨折

Treating adductor type of Monteggia by manual reduction plus plaster external fixation

目的:介绍一种复位儿童内收型孟氏骨折的手法,即掌击手法,并总结其治疗的效果。方法:对我院自2010年3月-2016年12月收治的60例儿童内收型孟氏骨折运用掌击手法进行复位,复位后屈肘不超过90°,前臂旋后位单纯长臂石膏后托或前臂夹板结合石膏后托固定。结果:60例患儿除5例失访,其余均获得随访。尺骨骨折均如期骨性愈合,按照Mackay等的功能评定标准分级,优54例,良1例。未出现骨筋膜室综合症,未发现桡骨头再脱位现象。结论:运用掌击手法复位结合石膏外固定治疗儿童内收型孟氏骨折,达到了良好的临床治疗效果,不失为一种安全有效地手法复位方法。

Objective:To introduce efficacy of manual reduction on adductor type of Monteggia.Methods:60 children with adductor type of Monteggia fractures were treated by manual reduction and plaster external fixation.Results:5 cases did not follow up;the other 55 cases were cured.According to the criteria of Mackay,54 cases were excellent,and 1 case was good,without compartment syndrome and dislocation of radial head.Conclusion:Manual reduction plus plaster external fixation on adductor type of Monteggia was effective.
