摘要
目的探析对老年股骨转子间骨折患者采取DHS(动力髋螺钉)与PFN(股骨近端髓内钉)治疗的临床效果。方法方便选取该院2012年5月—2017年5月期间接收的60例老年股骨转子间骨折患者,将其随机分为治疗组和对照组,对照组以DHS手术方式治疗,治疗组采取PFN手术方式治疗,比较两组髋关节功能优良率、并发症发生率、手术时用时、术中出血量情况、术后引流量及骨折愈合时间。结果治疗组髋关节功能优良率96.67%与对照组73.33%比较差异有统计学意义(P<0.05);治疗组术后并发症发生率3.33%与对照组16.67%比较差异有统计学意义(P<0.05);治疗组手术用时(51.6±8.3)min、术中出血量(140.7±10.3)m L、术后引流量(100.3±14.7)m L及骨折愈合时间(11.4±2.5)周与与对照组比较差异有统计学意义(P<0.05)。结论对老年股骨转子间骨折患者采取DHS与PFN治疗均具有效果,不过PFN更具微创性,优势更为突出,值得临床推广。
Objective This paper tries to study the clinical effect of DHS(dynamic hip screw)and PFN(proximal femoral nail)in elderly patients with intertrochanteric fractures.Methods 60 patients with intertrochanteric fractures from May 2012 to May 2017 were conveniently divided into the treatm ent group and the control group.The control group was treated with DHS.The treatm ent group was treated with PFN.The excellent rate of hip function,the incidence of com plications,the time of surgery,the am ount of bleeding during surgery,postoperative drainage and fracture healing time of the two groups were com pared.Results The excellent rate of hip function was 96.67%in the treatm ent group and 73.33%in the control group.The difference was statistically significant(P<0.05).The incidence of com plications in the treatm ent group was 3.33%and that in the control group was 16.67%,so the difference was significant(P<0.05).The surgical tim e was(51.6±8.3)m in,intraoperative blood loss was(140.7±10.3)m L,postoperative drainage volume was(100.3±14.7)m L and fracture healing time was(11.4±2.5)w eeks,significantly different from those in the control group(P<0.05).Conclusion DHS and PFN treatm ent in elderly patients with intertrochanteric fractures have certain effect,but PFN is more invasive,more prom inent,which is worthy of clinical promotion.
