内镜下金属钛夹止血治疗上消化道出血的疗效探讨

Therapeutic Effect of Endoscopic Titanium Clamp Hemostasis on Upper Gastrointestinal Bleeding

目的探讨内镜下金属钛夹止血治疗上消化道出血的临床疗效。方法方便选取云南省曲靖市师宗县人民医院消化内科2017年2—4月收治的非静脉曲张性上消化道出血患者45例为观察组,采用内镜下金属钛夹止血治疗,选取同期收治的40例采用内镜下喷洒药物治疗的非静脉曲张性上消化道出血患者为对照组,比较2组的止血情况、临床症状的消失情况以及并发症。结果观察组的止血总有效率高于对照组(97.78%VS 80.00%),再出血率低于对照组(6.67%VS 22.50%),差异有统计学意义(P<0.05)。观察组的呕血[(24.61±6.23)h VS(28.23±7.45)h]、潜血转阴[(2.56±0.46)d VS(4.31±0.47)d]、引流管变清[(3.26±0.55)d VS(4.87±0.62)d]、肠鸣音恢复时间短于对照组[(5.82±0.95)h VS(8.64±0.81)h],差异有统计学意义(P<0.05)。结论内镜下钛夹止血治疗上消化道出血的疗效优于内镜下喷洒药物,能够迅速缓解患者的临床症状。

Objective This paper tries to evaluate the clinical efficacy of endoscopic titanium clam p hem ostasis in the treatm ent of upper gastrointestinal bleeding.Methods 45 patients with non-v ariceal upper gastrointestinal bleeding treated in D epartm ent of G astroenterology of this hospital from February to April of 2017 were selected as the observation group and were given endoscopic titanium clam p hem ostasis.40 patients with non-variceal upper gastrointestinal bleeding in the same period treated by endoscopic spraying were selected as the control group.The hem ostasis,the disappearance of clinical symptoms and com plications were com pared betw een the two groups.Results The total effective rate of hem ostasis in the observation group was higher than that in the control group(97.78%vs 80.00%),and the rate of rebleeding was lower than that in the control group(6.67%vs 22.50%),the difference was statistically significant(P<0.05).H em atem esis in both groups were[(24.61±6.23)h vs(28.23±7.45)h_,occult blood clearance of[(2.56±0.46)d vs(4.31±0.47)d]and drainage tube cleaning of[(3.26±0.55)d vs(4.87±0.62)d],bowel sound recovery time of[(5.82±0.95)h vs(8.64±0.81)h]in the observation group,the difference was statistically significant(P<0.05).Conclusion The treatm ent of upper gastrointestinal bleeding by endoscopic titanium clip hem ostasis is better than endoscopic spraying,and it can relieve the clinical symptoms of the p atients rapidly.
