外固定支架结合VSD在下肢开放骨折合并软组织缺损治疗中的效果分析

Effect of External Fixator Combined with VSD in the Treatment of Open Fracture of Lower Limb Complicated with Soft Tissue Defect

目的探讨Vsd联合外固定架治疗下肢开发性骨折并皮肤软组织缺损的临床应用效果及安全性。方法方便选择2013年1月—2016年1月期间该院收治的35例患者作为研究对象,并随机分为观察组和对照组,对照组给予外固定支架治疗联合常规二期换药治疗,观察组给予VSD联合外固定架治疗,比较2种治疗方案的疗效和安全性。结果2组患者的治疗总有效率比较差异无统计学意义(P>0.05);在观察组中,患者骨折愈合时间(6.33±0.45)个月,对照组中,患者骨折愈合时间为(12.25±1.02)个月。观察组中浅表感染1例,骨髓炎1例,不良反应发生率为11.76%,对照组中浅表感染5例,深部感染2例,针道感染1例,骨髓炎1例,不良反应发生率为44.44%,呈现临床不均衡性差异有统计学意义(P<0.05)。结论 VSD联合外固定架治疗下肢开发性骨折并皮肤软组织缺损的疗效显著,能显著改善患者的预后质量,值得在临床上推广。

Objective This paper tries to investigate the clinical effect and safety in the treatm ent of lower lim b fracture developm ent VSD com bined with external fixation and skin soft tissue defect.Methods 35 patients in this hospital from January 2013 to January 2016 were convenient selected as the research objects and random ly divided into the observation group and the control group,the control group received external fixation com bined with conventional treatm ent of the second stage,the observation group was given VSD com bined with external fixation for the treatm ent,the efficacy and safety of two kinds of treatm ent were com pared.Results There were no significant difference of two groups in the total effective rate(P>0.05);in the observation group,the healing tim e of patients with fracture was(6.33±0.45)m onths,in the control group,patients with fracture healing time was(12.25±1.02)m onths.In the observation group,1 case was superficial infection,1 case was osteom yelitis,and the incidence of adverse reaction was 11.76%.In the control group,5 cases were superficial infection,2 cases were deep infection,1 case was needle channel infection,1 case was osteom yelitis,and the incidence of adverse reaction was 44.44%,the differences betw een the groups showed clinical disequilibrium(P<0.05).Conclusion The curative effect in the treatm ent of lower lim b fracture developm ent VSD com bined with external fixation and skin soft tissue defect is rem arkable,which can significantly improve the quality of patients.It is worthy of clinical promotion.
