手术治疗对前列腺增生疾病合并输尿管结石患者术后近远期性功能的影响研究

Effect of surgical treatment on the short and long-term function of patients with benign prostatic hyperplasia and ureteral calculi

目的:分析手术治疗前列腺增生疾病合并输尿管结石对患者术后近远期性功能的影响,为前列腺增生合并输尿管结石患者治疗方案选择提供参考。方法:选择我院2013年2月至2016年2月期间收治的72例前列腺增生疾病合并输尿管结石的患者作为研究对象,根据治疗方案的不同将其分为两组,药物保守治疗组(56例)和手术治疗组(62例)。比较两组患者入组时、完成治疗后的3个月、6个月、12个月的勃起功能评分表(IIEF-5)、射精功能评分表(CIPE)、国际前列腺症状评分(I-PSS)的变化。分析手术治疗对前列腺增生疾病合并输尿管结石患者性功能近远期影响。结果:两组患者年龄、前列腺增生临床症状表现、就诊时IIEF-5评分、CIPE评分、I-PSS评分比较差异均无统计学意义(P>0.05)。完成治疗后3个月手术组患者IIEF-5评分、CIPE评分、I-PSS评分均较治疗前明显降低(P<0.05),术后6个月恢复至术前水平,但治疗后12个月上述指标较术前有明显提升(P<0.05)。保守治疗组治疗后3个月、6个月、12个月较治疗前均无明显变化;手术治疗组患者腰痛、输尿管结石尿路梗阻症状和膀胱刺激症状改善时间均明显短于保守治疗组(P<0.05)。结论:手术治疗前列腺增生疾病合并输尿管结石可快速改善患者临床症状,对患者近期性功能产生一定的影响,但术后6个月性功基本恢复至术前,且术后12个月性功能较术前明显提升;保守治疗组性功能基本无变化。说明手术治疗前列腺增生合并输尿管结石对患者能提升患者远期性功能,具有一定的临床价值。

Objectives:To analyze the effect of surgical treatment for benign prostatic hyperplasia disease complicated with ureteral calculi on the short and long-term sexual function of patients,in order to provide reference for the selection of treatment.Methods:72 patients with prostatic hyperplasia complicated with ureteral calculi in our hospital from February 2013 to February 2016 were selected and,according to the different treatment options,were divided into two groups,drug treatment group(56 cases)and surgical treatment group(62 cases).Changes in the International Index of Erectile Function(IIEF-5),China Index of Ejaculatory function(CIPE)and International Prostate Symptom Score(I-PSS)between before treatment and 3 months,6 months,and 12 months after treatment were compared between the two groups.The effect of surgical treatment on the short and long-term sexual function of patients were compared.Results:There was no significant difference in the clinical symptoms,IIEF-5 score,CIPE score and I-PSS score between the two groups(P>0.05).In the surgical treatment group,the IIEF-5 score,CIPE score,I-PSS score was significantly lower at 3 months after treatment than before treatment(P<0.05).The difference was not significant at 6 months after treatment but was significant increased at 12 months after treatment(P<0.05).The average time for improvement in backache,irritation sign of bladder and ureteral calculi urinary obstruction symptom was significant shorter in the surgical treatment group than that in the drug treatment group(P<0.05).Conclusion:Surgical treatment of prostatic hyperplasia complicated with ureteral calculi can improve the clinical symptoms,which has a certain impact on the short and long-term sexual function of patients,significantly improved at postoperative 12 months,but there is no change in sexual function in the drug treatment group.The results reveal that surgical treatment of benign prostatic hyperplasia complicated with ureteral calculi can improve the long-term sexual function of patients with certain clinical value.
