IVF-ET术后1周患者血清β-HCG对妊娠结局的预测价值分析

Predictive value of serum β-HCG level in patients with pregnancy 1 week after IVF-ET

目的:探讨体外受精-胚胎移植后1周患者血清人绒毛膜促性腺激素(β-HCG)水平对妊娠结局的预测价值。方法:将我院符合条件的694例助孕周期且接受体外受精-胚胎移植手术的患者作为研究对象,回顾性分析IVF-ET手术后1周患者血清β-HCG值水平情况。结果:单胎妊娠组β-HCG值与多胎妊娠组相比较,差异明显,具有统计学意义(P<0.05);生化妊娠流产组的β-HCG值与临床妊娠组比较,差异具有统计学意义(P<0.05);早期临床妊娠流产组的β-HCG值与继续妊娠组比较差异具有统计学意义(P<0.05);血清β-HCG值低于100U/L时可以作为不良妊娠的判断限值,且阳性预测价值是83.6%,阴性预测价值是85.6%;血清β-HCG值高于1000U/L对多胎妊娠的阳性预测价值是68.9%,阴性预测价值是81.3%。结论:IVF-ET手术后1周单次血清β-HCG值可以作为预测妊娠结局的有效指标,利于临床诊断。

Objectives:To study the predictive value of serum chorionic gonadotropin(β-HCG)level in pregnancy outcome at a week after in vitro fertilization and embryo transfer(IVF-ET).Methods:The level of serumβ-HCG in 694 patients was analyzed retrospectively after IVF-ET.Results:The difference inβ-HCG between single pregnancy group and multiple pregnancy group was statistically significant(P<0.05).The difference inβ-HCG between biochemical pregnancy abortion group and clinical pregnancy group was statistically significant(P<0.05).The difference inβ-HCG between the early pregnancy group and continuing pregnancy group was statistically significant(P<0.05).Serumβ-HCG value of less than 100U/L could be used as limiting value to determine the adverse pregnancy.Positive predictive value was 83.6%,and negative predictive value was 85.6%;The predicted value of serumβ-HCG value higher than 1000U/L for positive multiple pregnancy was 68.9%and the negative predictive value was 81.3%.Conclusion:The level of serumβ-HCG 1 week after IVF-ET can be used as an effective index to predict the outcome of pregnancy.
