妇科老年患者卵巢癌术后尿路感染的临床分析

Clinical Analysis of Postoperative Urinary Tract infection in Elderly Patients with Ovarian Cancer After Gynecologic Surgery

目的探讨妇科老年患者卵巢癌术后尿路感染的临床分析。方法对我院妇科2013年1月~2015年11月收治的150例行卵巢癌术老年患者临床资料进行回顾性分析。结果发生尿路感染65例,占比43.3%。患者临床表现为:尿路刺激征(尿频、尿急、尿痛)、尿潴留、尿道口瘙痒、尿液浑浊、血尿、发热、身躯叩击痛、腰部疼痛,阳性率分别为47.7%、30.8%、23.1%、16.9%、15.4%、10.8%、7.7%、4.6%。其中尿路刺激征(尿频、尿急、尿痛)居于第一位,较为多见,部分患者多项临床症状同时存在;细菌培养结果显示,包括革兰阳性菌41株、革兰阴性菌30株、真菌2株,占比分别为:56.2%、41.1%、2.7%;两组患者导管留置时间、插管次数、住院时间对比差异具有统计学意义(P<0.05)。结论妇科老年患者卵巢癌术后尿路感染易感因素包括留置时间、插管次数、住院时间等,临床实践中要针对性做好护理工作,减少发生尿路感染的机率。

Objective To investigate the clinical features of postoperative urinary tract infection in elderly patients with ovarian cancer after gynecologic surgery.Methods The clinical data of 150 elderly patients with ovarian cancer treated in our hospital from January 2013 to November 2015 were retrospectively analyzed.Results 65 cases of urinary tract infection occurred,which accounted for 43.3%of total.The clinical manifestations included urinary tract irritation(frequent urination,urgent urination,odynuria),urinary retention,urethral itching,urine turbidity,hematuria,fever,body percussion pain,waist pain,and corresponding positive rates was 47.7%,30.8%,23.1%,16.9%,15.4%,10.8%,7.7%,4.6%,respectively.The urinary tract irritation syndrome(frequent urination,urgent urination,odynuria)was in the first place and more common.Some patients complained more than one symptom;Bacterial culture results showed that there were 41 strains of gram positive bacteria,30 strains of gram negative bacteria and 2 strains of fungi,which accounted for 56.2%,41.1%,2.7%,respectively(P<0.05).The differences were statistically significant between the two groups when compared of the catheter indwelling time,intubation times and the length of hospital stay(P<0.05).Conclusion The predisposing factors of postoperative urinary tract infection in gynecological elderly patients with ovarian cancer include catheter indwelling time,intubation times,the length of hospital stay and so on.In clinical practice,targeted nursing care should be performed to reduce the risk of urinary tract infection.
