当归四逆汤加减联合TDP治疗原发性痛经临床疗效及子宫动脉血流动力学影响研究

Analysis of clinical treatment effect and uterine hemodynamic changes of primary dysmenorrhea treated by Danggui Sini decoction and TDP

目的:分析当归四逆汤加减与特定电磁波治疗仪(TDP)治疗原发性痛经临床治疗效果及子宫血流动力学变化影响。方法:选取我院收治的原发性痛经患者随机分为对照组和观察组。对照组采用布洛芬缓释胶囊治疗,观察组采用当归四逆汤加减联合TDP治疗,分析两组患者治疗效果,对子宫动脉血流动力学参数和各细胞因子水平影响。结果:治疗前两组小腹疼痛或坠胀、腰酸胀痛、手脚冰冷、腹泻等症状积分和子宫动脉搏动指数、阻力指数、收缩期峰值/舒张期峰值比较无统计学差异(P>0.05)。治疗后观察组主要症状体征积分、子宫动脉血流动力学参数水平均低于对照组(P<0.05)。治疗前两组hs-CRP、IL-6、HIS、PGF_(2α)、Fib水平比较无统计学差异(P>0.05)。治疗后观察组炎症因子、HIS、PGF2α、Fib水平均低于对照组(P<0.05)。结论:当归四逆汤加减联合TDP治疗原发性痛经疗效显著,可改善患者子宫动脉血流动力学,减轻机体的微炎症反应程度,调节患者血清HIS、PGF_(2α)水平。

Objective:To analyze the clinical treatment effect and uterine hemodynamic changes of primary dysmenorrhea treated with Danggui Sini decoction and specific electromagnetic wave therapeutic apparatus(TDP).Methods:In our hospital patients with primary dysmenorrhea in 122 cases,according to randomly divided into control group and observation group.The control group was treated with ibuprofen sustained-release capsules treatment,the observation group were treated with angelica four inverse decoction combined with TDP treatment,analysis of treatment effect of two groups of patients,effects on uterine artery blood flow parameters and the the levels of cytokines.Results:Before treatment,two groups of patients with abdominal pain and distention,backache pain,cold hands and feet,diarrhea and other symptoms,uterine artery pulsatility index,resistance index,peak systolic/diastolic peak had no significant difference(P>0.05).After the treatment,the observation group of patients with symptoms of main body syndrome integral,uterine artery hemodynamic parameters were significantly lower than that of the control group(P<0.05).The two groups of patients before treatment,hs-CRP,IL-6,HIS,PGF 2α,no significant differences between the levels of Fib(P>0.05).After treatment,the observation group of patients with inflammatory factors,HIS,PGF 2α,Fib water was lower than that of the control group(P<0.05).Conclusion:Danggui Sini decoction combined with TDP treatment of primary dysmenorrhea curative effect is remarkable,can improve the patient's uterine hemodynamics,reduce the degree of micro inflammatory reaction,regulate the serum HIS,PGF 2αlevel.
