针药结合治疗阿片类药物相关性便秘临床研究

Clinical observation on the treatment of opioid related constipation with acupuncture and medicine

目的:研究针药结合治疗阿片类药物相关性便秘及西药治疗的对比结果。方法:选取我院中医内科和宁养院门诊及住院患者共100例为研究对象,以数字法随机分为针灸组、中药汤剂组、中药汤剂结合针灸组、西药组、安慰剂组共五组,每组各20例。针灸组给予针灸足三里及四关穴治疗,中药汤剂组给予中药温脾通关汤口服,中药汤剂结合针灸组给予中药温脾通关汤结合针灸足三里及四关穴,西药组给予聚乙二醇散口服,安慰剂组给予安慰剂。观察各组治疗7d、14d及治疗后14d的便秘情况及癌痛缓解情况。结果:治疗第7天、14天,针药合治组便秘评分分别为(15.50±4.25)分、(7.60±2.64)分。癌痛评分分别为(3.45±1.67)分、(2.10±1.55)分(P<0.01)。结论:针药并治组在治疗第7天、14天及治疗后14天的便秘改善及癌痛改善优于其他组。对阿片类药物相关性便秘,针药并治临床疗效较好,提高患者的生活质量,具有较大的临床使用意义。

Objective:To compare the therapeutic effects of acupuncture and medicine on the treatment of opioid related constipation and western medicine.Methods:In our hospital of traditional Chinese medicine and the hospice out-patient and hospitalization in patients with a total of 100 cases as the research object,the numbers were randomly divided into acupuncture group,Chinese medicine group,Chinese medicine decoction combined with acupuncture group,western medicine group and the placebo group five group,20 cases in each group.The acupuncture group received acupuncture Zusanli and four acupuncture treatment,traditional Chinese medicine decoction group was treated with traditional Chinese medicine Wenpi decoction combined with Tongguan decoction orally,the acupuncture group was treated with traditional Chinese medicine Wenpi decoction combined with acupuncture Zusanli clearance and four Guan,the western medicine group were given oral placebo group of polyethylene glycol,given placebo.The constipation and pain relief of 7 days,14 days and 14 days after treatment were observed.Results:After seventh days and 14 days of treatment,the scores of constipation were(15.50±4.25)and(7.60±2.64)respectively,and the scores of cancer pain were(3.45±1.67)and(2.10±1.55)respectively(P<0.01).Conclusion:The improvement of constipation and the improvement of cancer pain in seventh days,14 days and 14 days after treatment were superior to those of other groups.The correlation between opioid constipation,acupuncture and medicine clinical curative effect is better,improve the quality of life of patients,is of great significance for clinical use,is worthy of recommendation.
