玉泉汤联合丹参注射液治疗口腔黏膜下纤维性变疗效及对患者血清白细胞介素6、转化生长因子β1、血液流变学指标的影响

Effect of Yuquan decoction combined with Danshen injection on oral submucous fibrosis and its influences onserum interleukin-6,transforming growth factor-β1 and hemorheological parameters

目的观察玉泉汤联合丹参注射液治疗口腔黏膜下纤维性变(OSF)的临床疗效及对患者血清白细胞介素6(IL-6)、转化生长因子β1(TGF-β1)水平、血液流变学指标的影响。方法将86例OSF患者按随机数字表法分为2组。对照组43例应用丹参注射液治疗;治疗组43例应用玉泉汤联合丹参注射液治疗。2组均治疗3个月。比较2组临床疗效,记录2组治疗前后疼痛视觉模拟评分法(VAS)评分,统计2组治疗前后张口度和口腔黏膜病损面积,检测2组治疗前后血清IL-6、TGF-β1水平及血液流变学指标。结果治疗组总有效率88.37%,对照组总有效率69.77%,治疗组临床疗效优于对照组(P<0.05)。2组治疗后疼痛VAS评分均较本组治疗前降低(P<0.05);治疗后治疗组疼痛VAS评分低于对照组(P<0.05)。2组治疗后张口度均较本组治疗前升高(P<0.05),口腔黏膜病损面积均较本组治疗前降低(P<0.05);治疗后治疗组张口度高于对照组(P<0.05),口腔黏膜病损面积低于对照组(P<0.05)。2组治疗后血清IL-6、TGF-β1水平均较本组治疗前降低(P<0.05);治疗后治疗组血清IL-6、TGF-β1水平均低于对照组(P<0.05)。2组治疗后血浆黏度、全血黏度(低切)及纤维蛋白原均较本组治疗前降低(P<0.05);治疗后治疗组血浆黏度、全血黏度(低切)及纤维蛋白原均低于对照组(P<0.05)。结论玉泉汤联合丹参注射液治疗OSF疗效满意,可明显降低患者血清IL-6、TGF-β1水平,改善血液流变学指标。

Objective To observe the clinical effects of Yuquan decoction combined with Danshen injection on oral submucous fibrosis(OSF)and its influences on serum interleukin-6(IL-6),transforming growth factor-β1(TGF-β1)and hemorheological parameters.Methods 86 patients with OSF were randomly divided into two groups.43 cases in control group were treated with Danshen injection,and 43 cases in treatment group were treated with Yuquan decoction combined with Danshen injection,and the two groups were treated for 3 months.The clinical efficacy was compared between the two groups,the visual analogue scale(VAS)scores before and after treatment were recorded in the two groups,the mouth opening amplitude and the oral mucosal lesion area before and after treatment in the two groups were evaluated,the levels of serum IL-6,TGF-β1 and hemorheological parameters were detected before and after treatment in the two groups.Results There were statistical differences on the total effective rate between the treatment group(88.37%)and control group(69.77%,P<0.05).The clinical efficacy in treatment group was better than that in control group.The VAS scores after treatment were reduced in two groups(P<0.05),and which in treatment group was lower than in control group(P<0.05).The mouth opening amplitude after treatment were increased in two groups(P<0.05),and the oral mucosal lesion area was reduced(P<0.05).The mouth opening amplitude after treatment in treatment group was higher than in control group(P<0.05),and the oral mucosal lesion area in treatment group was lower than in control group(P<0.05).The levels of serum IL-6 and TGF-β1 after treatment were reduced in two groups(P<0.05),and which in treatment group was lower than in control group(P<0.05).The plasma viscosity,whole blood viscosity(low shear)and fibrinogen were reduced in two groups(P<0.05),and which in treatment group was lower than in control group(P<0.05).Conclusion Yuquan decoction combined with Danshen injection has a satisfactory curative effect on OSF,and it can significantly reduce the levels of serum IL-6 and TGF-β1 and improve hemorheology.
