两种颈椎椎管扩大成形术治疗多节段颈椎退行性疾病的临床疗效分析

Clinical effect analysis on two kinds of operation of cervical vertebra canal expand plasty for treatment of cervical degenerative disease

目的分析两种颈椎椎管扩大成形术治疗多节段颈椎退行性疾病的临床疗效。方法选择2015年6月~2016年9月我院收治的74例行颈椎椎管扩大成形术患者为研究对象,按手术方法的不同随机分为实验组和对照组各37例,实验组使用单开门式椎管成形术行手术治疗,对照组使用双开门式椎管成形术行手术治疗,观察比较两组患者的手术时间、术中出血量、随访时间以及术后并发症发生情况,并采用日本骨科协会评估治疗(JOA)评分进行评定两组患者术前、术后1周和术后2个月的治疗效果。结果经治疗后,实验组患者的手术时间、术中出血量、随访时间以及并发症发生率均低于对照组,组间比较差异有统计学意义(P<0.05);实验组患者术后的JOA评分显著优于对照组,差异有统计学意义(P<0.05)。结论单开门式椎管成形术治疗多节段颈椎退行性疾病的手术时间短,术中出血量少,并发症发生率低,能有效改善患者的症状,提高治疗效果。

Objective To analyze the clinical curative effect on two kinds of cervical vertebra canal expansion plasty for treatment of multiple segmental construction of cervical degenerative disease. Methods 74 patients with cervical spinal canal expanded from June 2015 to September 2015 of our hospital,according to the different method of surgery were randomly divided into experimental group and the control group(each n=37).The experimental group was used the surgery of single door type spinal canal plasty,and the control group was used the double door type.The patients of two groups were be observed and compared on the operation time,intraoperative blood loss,follow-up time and postoperative complications.Two groups of patients were used the Japanese orthopaedic association(JOA)to evaluate the condition before and after 1 week and 2 months operation. Results After treatment,the patients of experimental group with operation time,intraoperative blood loss,and follow-up time and complication rates were lower than the control group(P<0.05).Postoperative JOA score of experimental group was significantly better than the control group(P<0.05). Conclusion The single door type of vertebral canal expanded plasty for treatment of cervical degenerative disease will be shorter the operation time,less the intraoperative blood loss,low the complication rate,effectively improve the patient's symptoms,and improve the effect of treatment.
