川崎病患儿治疗前后血浆FIB、CRP水平和血小板计数变化与冠状动脉病变的相关性

Association of the changes of plasma Fib, CRP levels and the platelet count and coronary artery lesions before and after treatment with Kawasaki disease

目的观察川崎病患儿治疗前后血浆纤维蛋白原(FIB)、C反应蛋白(CRP)水平和血小板计数的变化,探讨FIB、CRP和血小板计数与冠状动脉病变(coronary artery lesions,CAL)的相关性。方法将本院2012年6月至2017年3月确诊为川崎病的患儿48例纳入观察组,并根据切面超声心动图对冠状动脉的检测结果将其分为CAL组(27例)和NCAL组(21例),观察组患儿采用静脉注射丙种球蛋白和口服阿司匹林治疗。选取本院同期30例门诊体检健康儿童纳入对照组,检测比较各组患儿血浆FIB、CRP水平及血小板计数。结果观察组患儿治疗前血浆FIB、CRP水平及血小板计数均明显高于对照组(P<0.05),CAL组患儿上述三项指标均明显高于NCAL组(P<0.05)。治疗后观察组患儿上述三项指标均显著低于治疗前(P<0.05),且NCAL组患儿上述指标水平较CAL组下降幅度更大(P<0.05)。结论血浆FIB、CRP水平及血小板计数与CAL及病情发展密切相关,是评估川崎病治疗效果的重要指标。

Objective To observe the changes of disease of plasma fibrinogen(FIB),C reactive protein(CRP)levels and platelet count in Kawasaki disease(KD)children before and after treatment,to investigate the association of FIB,CRP and platelet count with coronary artery lesions(CAL).Method From June 2012 to March 2017,48 cases of KD children in our hospital were selected as observation group,according to ultrasound heartbeat detection results of coronary artery,they were divided into CAL group(n=27)and NCAL group(n=21),observation group were treated by intravenous immunoglobulin and aspirin treatment.During the same time,30 healthy children in our hospital were selected as the control group.The plasma levels of FIB,CRP and platelet count were compared.Result Before treatment,plasma FIB,CRP levels and platelet count in observation group were significantly higher than control group(P<0.05);among them,the three indexes of CAL group was higher than that of NCAL group(P<0.05),observation group after treatment of the three indexes decreased significantly(P<0.05),among them,NCAL group was much lower than that in CAL group(P<0.05).Conclusion The plasma FIB,CRP levels and platelet counts are closely related to CAL and the development of the disease.They are important indexes to evaluate the curative effect of KD.
