DHS与PFN治疗股骨粗隆间骨折的临床效果观察

Clinical effect observation of DHS and PFN in the treatment of femoral intertrochanteric fracture

目的分析动力髋螺钉(DHS)与股骨近端髓内钉(PFN)治疗股骨粗隆间骨折患者的临床效果。方法选取我院2016年1月至2016年12月收治的50例股骨粗隆间骨折患者作为研究对象,根据不同治疗方式将其分为研究组(25例)与对照组(25例)。研究组行PFN治疗,对照组行DHS治疗。比较两组髋关节功能改善优良率与术后骨折愈合时间、术后负重时间、术中出血量、手术时间。结果研究组手术之后的髋关节功能优良率为96.00%,高于对照组的68.00%(P<0.05);研究组的术后骨折愈合时间、术后负重时间、手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。结论 PFN应用于股骨粗隆间骨折患者治疗中的效果优于DHS,可缩短患者的手术时间、术后负重时间、术后骨折愈合时间,减少术中出血量,进一步促进髋关节功能的恢复,值得临床推广运用。

Objective To analyze the clinical effect of dynamic hip screw(DHS)and proximal femoral nail(PFN)in patients with intertrochanteric fractures.Methods Fifty cases of patients with intertrochanteric fracture in our hospital from January 2016 to December 2016 were selected as the study subjects,and were divided into study group and control group according to different treatment methods,with 25 cases in each group.The study group was treated with PFN and the control group was treated with DHS.The improvement of hip joint function,fracture healing time,postoperative weight-bearing time,intraoperative blood loss,operation time were compared in the two groups.Results The excellent and good rate of hip function after operation in the study group was 96.00%,which was higher than 68.00%in the control group(P<0.05);The fracture healing time,postoperative weight-bearing time and operation time in the study group were shorter than those in the control group,the blood loss in the study group was less than that in the control group,the differences were statistically significant(P<0.05).Conclusion PFN in patients with femoral intertrochanteric fracture hae better effect than DHS,it can shorten the operation time,postoperative weight-bearing time,postoperative fracture healing time,reduce the amount of bleeding,and promote the recovery of hip function,which is worthy of promotion and application in clinic.
