瑞芬太尼用于全身麻醉术后患者围拔管期的临床效果观察

Clinical effect observation of remifentanil in extubation period of patients with general anesthesia

目的探讨瑞芬太尼用于全身麻醉术后患者围拔管期的临床效果。方法选取2015年1月至2016年12月在我院行全身麻醉手术的100例患者为研究对象,遵循单盲随机分配原则将患者分为两组,各50例。对照组静脉输注生理盐水辅助麻醉,观察组静脉输注小剂量瑞芬太尼辅助麻醉。比较两组患者术后意识恢复时间、拔管时间、各时间点血压及心率水平和不良反应发生率。结果观察组患者的术后意识恢复时间和拔管时间均明显短于对照组(P<0.05);拔管前,两组患者的血压、心率比较,差异不显著(P>0.05);拔管后,观察组患者的血压、心率无明显变化(P>0.05),对照组患者的血压、心率均显著高于拔管前(P<0.05)。观察组的不良反应发生率均明显低于对照组(P<0.05)。结论在全身麻醉手术中应用小剂量瑞芬太尼可有效减少术后围拔管期不良反应,维持血压及心率稳定,促使患者尽快苏醒。

Objective To study and explore the clinical effect of remifentanil in extubation period of patients with general anesthesia.Methods From January 2015 to December 2016,100 cases of general anesthesia surgery patients in our hospital were selected as the research objects and divided into two groups according to randomized single blind,with 50 cases in each group.The control group was given infusion of saline assisted anesthesia,the observation group was given infusion of small dose remifentanil anesthesia.The consciousness recovery time,extubation time,the blood pressure and heart rate at each time and the incidence of adverse reactions were compared between the two groups.Results Compared with the control group,the postoperative consciousness recovery time and extubation time in the observation group were significantly shorter(P<0.05).Before extubation,there were no significant differences in the blood pressure and heart rate between the two groups(P>0.05);after extubation,the blood pressure and heart rate in the observation group had no obvious change(P>0.05),and those in the control group were higher than before extubation(P<0.05).The incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The small dose remifentanil used in general anesthesia surgery can effectively reduce the incidence of adverse reactions during the extubation period,maintain the stability of the blood pressure and heart rate,and make patients to wake up as soon as possible.
